葡萄膜黑色素瘤术后自体细胞因子诱导的杀伤细胞治疗效果观察

Clinical effects of autologous CIK treatment for postoperative uveal melanoma

目的探讨葡萄膜黑色素瘤术后自体细胞因子诱导的杀伤细胞(cytokine-induced killer cell,CIK)治疗的效果。方法对我院2005年1月至2009年1月收治的38例葡萄膜黑色素瘤术后患者进行分析,18例患者未进行抗肿瘤治疗,设为对照组;20例患者进行自体CIK治疗,设为CIK治疗组。观察2组1a、3a内复发率及3a内生存率,并观察CIK治疗组治疗前及治疗3个月后患者外周血CD3+、CD4+、CD8+、CD16+、CD56+细胞情况。结果对照组患者1a内复发3例(16.67%),3a内复发9例(50.00%),3a内生存14例(77.78%);CIK治疗组1a内复发2例(10.00%),3a内复发6例(30.00%),3a内生存18例(90.00%)。2组间1a内复发率比较,差异无统计学意义(P>0.05);3a内复发率与生存率比较,差异均有统计学意义(均为P<0.05)。CIK治疗3个月后,患者外周血中T细胞(CD3+、CD4+、CD8+)和NK细胞(CD16+、CD56+)均显著上升,与治疗前比较,差异均有统计学意义(P<0.05或0.01)。结论葡萄膜黑色素瘤术后自体CIK治疗可获得较好的疗效。

Objective To investigate the efficacy of autologous cytokine-induced killer cell（CIK） treatment for postoperative uveal melanoma.Methods A retrospective study was done on 38 postoperative patients with uveal melanoma from January 2005 to January 2009 in our hospital,18 cases（control group） were not treated,20 cases（treatment group） with autologous CIK treatment.The recurrence rates of two groups within 1 year and 3 years were observed,and the survival rate of two groups within 3 years was recorded.The CD3＋ and CD4＋,CD8＋,CD16＋ and CD56＋ cell of treatment group before treatment and 3 months after treatment was observed.Results In treatment group,the recurrence rates were 16.67%（3 cases） and 50.00%（9 cases） within 1 year and 3 years,respectively,the survival rates were 77.78%（14 cases） within 3 years;In control group,the recurrence rates were 10.00%（2 cases） and 30.00%（6 cases） within 1 year and 3 years,respectively,the survival rate was 90.00%（18 cases） within 3 years;There was no statistical difference in recurrence rate within 1 year between two groups（P〉0.05）,statistical differences in recurrence rate and survival rate within 3 years between two groups（both P〈0.05）.After CIK treatment,the T cells（CD3＋ and CD4＋,CD8＋） and NK cells（CD16＋,CD56＋） in peripheral blood were increased significantly,compared with that before treatment,there were statistical differences（P〈0.05 or 0.01）.Conclusion The autologous CIK treatment for postoperative uveal melanoma can achieve a good clinical effect.
