胃肠道间质瘤的异生血管CT表现及临床应用

Gastrointsetinal stromal tumors heterogenesis:CT findings and its clinical application

目的:探讨胃肠道间质瘤的异生血管CT表现及临床中的应用。方法:回顾性分析本院经临床手术病理证实为有异生血管的胃肠道间质瘤的患者14例,CT增强表现有异生血管给瘤体供血,并参考相关文献分析。结果:单发团块影11例,两个或多个团块3块,9例肿块型囊变;肝左动脉供血的1例空肠间质瘤,2例胃间质瘤;腹腔干分支供血的2例十二指肠间质瘤、1例胃间质瘤及1例肠系膜间质瘤;脾动脉供血的1例肠系膜间质瘤;髂外动脉分支供血的3例回肠间质瘤;髂内动脉供血的2例结肠间质瘤。结论:胃肠道间质瘤的血供十分丰富,可由本脏器的原始血管供血,同时还可存在瘤体以外脏器的血管供血,在较大肿瘤的定位时,有异生血管形成时,以胃肠道间质瘤为多见,同时有异生血管的肿瘤以腔外型最常见,而绝大多数以恶性为主,同时也为临床术前手术方式提供可靠的依据。

Objective：To investigate the difference, of gastrointestinal stromal tumor angiogenic CT findings and clinical application. Methods： Analyzes retrospectively by clinical pathologically confirmed to have different angiogenie gastrointestinal stromal tumors of 14 cases, CT enhancement to the performance of different angiogenic tumor blood supply, and withreference to relevant literature. Results：Single mass was showed 11 cases, two or more 3, the solid part of the nodule or mass was significantly enhanced. 9 cases of cystic tumor; left hepatic artery in one case of jejunal stromal tumor, 2 cases gastric stromal tumor; are cavity shape, are vicious; left gastric artery in one case of jejunal stromal tumor, malignant, cavity shape; celiac branch of the blood supply of 2 cases of duodenal stromal tumor, 1 case gastric stromal tumor and 1 case of mesenteric stromal tumor, 4 cases were malignant, the cavity shape in 3 cases; splenic artery in one case of mesen-teric stromal tumor, malignant, cavity shape external iliac artery blood supply in 3 cases of ileumstromal tumor. 1 case of benign, 2 were malignant, and 1 wall-type, 2 cases of cavity shape iliac artery in 2 cases of colonic stromal tumor, 2 cases were malignant, cavity shape. Conclusion：Gastrointestinal stromal tumor of the blood supply is very rich, by the organs of the original blood vessels, but can also exist outside the organ of blood vessels feeding the tumor, tumor location in the larger, when blood vessel formation, with gastrointestinal stromal tumor is more common, extraluminal vascular tumors the most common,and most of the vicious-based, but also for clinical preoperative provide a reliable basis.
