U100双频激光脉冲碎石术与体外冲击波碎石术治疗输尿管上段结石的疗效比较 被引量:4

摘要 目的探讨U1OO双频激光脉冲碎石术对比体外冲击波碎石术(ESWL)治疗输尿管上段结石的疗效差异。方法对经U1OO双频激光和体外冲击波碎石术这两种碎石方法治疗输尿管上段结石的疗效进行比较。结果治疗时间U1OO双频激光组平均为(34±1.9)min,ESWL组治疗时间平均为(33±2.7)min(P>0.05)。U1OO双频激光组结石排净率91.03%(71/78),ESWL组为82.68%(105/127)(P<0.05);UlOO双频激光组结石逃逸到肾盂发生率6.4%(5/78),ESWL组为8.6%(11/127)(P<0.05);U1OO双频激光组术后肉眼血尿29例,发生率为37.18%(29/78),而ESWL组术后肉眼血尿82例,发生率为64.57%(82/127)(P<0.05)。其余并发症U1OO双频激光组为2.56%(2/78);ESWL组为6.30%(8/127)(P<0.05)。结论 U100双频激光脉冲碎石术在输尿管上段结石的治疗中优于体外冲击波碎石术,具有成功率高、安全、并发症少等优点。 [ Objective ] To investigate the therapeutic effect between frequency-doubled double-pulse laser and extracorporeal shock wave lithotripsy (ESWL) for upper nreteral calculi. [Methods ] To comparatively study the time length of operation, stone-free rate, cure rate and complication rate of the two methods for treatment of upper ureter- al calculi. To summarize the advantages and disadvantages of the two treatments. [Results] The mean operation time in frequency-doubled double-pulse laser group was (34±1.9) rain, in ESWL group was (33±2.7) min (P 〉0.05). The stone-free rate in frequency-doubled double-pulse laser group was 91.03% (71/78) and was 82.68% (105/127) in ESWL group (P 〈0.05). The stone migration rate in frequency-doubled double-pulse laser group was 6.40% (5/78) while in ESWL group was 8.60% (11/127) (P 〈0.05). The rate of gross hematuria was 37.18% (29/78) in fre- quency-doubled double-pulse laser group and 64.57% (82/127) in ESWL group (P 〈0.05). The other complications in frequency-doubled double-pulse laser group were 2.56% (2/78) while 6.30% (8/127) in ESWL group. [ Conclusions ] The frequency-doubled double-pulse laser for upper nreteral calculi is markedly surpassed the extracorporeal shock wave lithotripsy. And it's an efficient safe method with fewer complications for the treatment of upper ureteral
