多西他赛联合顺铂治疗鼻咽癌放疗后远处转移的临床观察

Clinical Observation of Docetaxel and Cisplatin in the Treatment of Distant Metastasis of Nasopharyngeal Carcinoma After Radiotherapy

目的探讨多西他赛联合顺铂治疗鼻咽癌放疗后远处转移的临床疗效和毒副反应。方法将80例发生放疗后远处转移鼻咽癌患者分为研究组和对照组,每组40例。研究组40例患者给予多西他赛联合顺铂治疗,对照组40例患者仅给予对症支持治疗。治疗后比较观察2组患者的临床疗效,同时记录研究组的毒副反应。结果研究组总有效率为72.5%,高于对照组的5.0%(P<0.05)。研究组主要毒副反应有胃肠道反应、骨髓抑制等,大多为轻度。研究组生存时间为≤1 a者5例(12.5%),>1～2 a者20例(50.0%),>2～3 a者13例(32.5%),>3 a以上者2例(5.0%);对照组生存时间为≤1 a者27例(67.5%),>1～2 a者12例(30.0%),2～3 a者1例(2.5%),2组总体比较差异有统计学意义(P<0.05)。结论多西他赛联合顺铂治疗鼻咽癌放疗后远处转移临床疗效较好,毒副反应可耐受。

Objective To investigate the efficacy and toxicities of docetaxel and cisplatin in the treatment of the distant metastasis of nasopharyngeal carcinoma after radiotherapy. Methods Eighty cases of distant metastases of nasopharyngeal carcinoma after radiotherapy were divided into two groups, the 40 cases of the study group received the docetaxel and cisplatin chemotherapy, and the 40 cases of the control group received the supporting therapy. The efficacy of the two groups were compared, and the toxicities of the study group were evaluated too. Results The total response rate was 72.5% in the study group, and was 5.0% in the control group （ P 〈0.05 ）. The main toxicities of the study group were low-grade gastrointestinal reactions and marrow suppression. In the study group, the survival time of 5 patients （12.5%）was 1 year, 1 to 2 years 20 patients （50.0%）, 2 to 3 years 13 patients （32.5%）, 〉 3 years 2 patients （5.0%） ; in the control group, 1 year 27 patients （67.5 % ）, 1 to 2 years 12 pa-tients （30.0%）, 2 to 3 years patient 1 （2.5 % ） （ P 〈 0.05 ）. Conclusion Docetaxel and eisplatin is effective in the treatment of the distant metastasis of nasopharyngeal carcinoma after radiotherapy, and the toxicities can be tolerated.
