无创血流动力学监测在全肺灌洗患者中的应用 被引量:2

摘要 目的观察肺泡蛋白沉积症(PAP)患者行大容量全肺灌洗过程中血流动力学的变化。方法对14例行全肺灌洗术的PAP患者予无创血流动力学监测系统(Bioz.com)监测,观察HR、MAP、CVP以及CO、SV、TFC、ACI、SVRI变化并做统计分析。结果每次灌洗液注入肺时引起血流动力学改变,表现为CVP、TFC升高,HR、MAP、SV(P<0.05),灌洗液吸出后改善。随着灌洗次数增加,血流动力学抑制更明显。结论无创血流动力学监测系统(Bioz.com)在肺灌洗患者中监测的血流动力学参数在每次灌洗液灌入前后均出现周期性波动,为肺灌洗患者的监测和治疗提供可靠依据。 Objective To observe the changes of hemodynamics in pulmonary alveolar proteinosis (PAP) patients dur- ing whole lung lavage. Methods 14 patients of whole lung lavage were observed HR, MAP, CVP, CO, SV, TFC, ACI and SVRI. Results The data of hemodynamies were changed when lavage fluid instilled into the lung. CVP, TFC increased ( P 〈 0. 05 ), but HR, MAP, SV decreased ( P 〈 0. 05 ), Hemodynamics data resumed after fluid sucked out. Hemodynamics data were obviously inhibited in the fifth and tenth lavage, and resumed after two lung ventilation ( P 〈 0. 05 ) . Conclusion He- modynamics data fluctuate periodically before and after fluid instilled into the lung. Non-invasive hemodynamics monitoring sup- port an useful monitor for patients in whole-lung lavage.
