腰硬联合麻醉用于妊娠期高血压剖宫产的临床体会

Experiences on the clinical application of combined subarachnoid epidural anesthesia at the patients with pregnancy induced hypertension for cesarean

目的探讨腰硬联合麻醉在妊娠期高血压产妇剖宫产手术中的应用效果。方法选取妊娠期高血压拟行剖宫产产妇356例,分为腰硬联合组(E组)和硬膜外组(S组),观察两组的麻醉起效时间、麻醉效果分级评价及麻醉对全身的影响和新生儿Apgar评分、麻醉并发症等。结果腰硬联合组麻醉起效时间显著优于硬膜外组(P<0.05),麻醉效果评价明显优于硬膜外组(P<0.05),而两组在低血压、恶心呕吐的发生率、麻黄素和阿托品的使用方面和新生儿Apgar评分、麻醉并发症等方面无明显差异(P>0.05)。结论腰硬联合麻醉起效快,效果确切,能安全有效地用于妊高征剖宫产手术麻醉。

Objective To observe the clinical application of combined subarachnoid epidural anesthesia at the patients with pregnancy induced hypertension for cesarean. Methods A total of 356 patients with pregnancy induced hypertension were randomly divided into two groups （group E and group S）. To observe the two groups of anesthesia work time, anesthesia effect, anesthesia grading evaluation, the Apgar score of newborn and the anesthesia complication. Results The combined subarachnoid epidural anesthesia group works time was significantly better than epidural group （P 〈 0.01）, the effect of anesthesia is better than epidural group （P 〈 0.05 ）, the two groups in low blood pressure, the incidence of nausea, vomiting, ephedrine and atropine the use of and newborn Apgar score, anesthesia complications had no obvious difference （P 〉 0.05）. Conclusion The combined subaraehnoid epidural anesthesia work fast, exact effect, can safely and effectively used for ce- sarean performed on patients with pregnancy induced hypertension.
