糖尿病肾病患者血清CRP、HbA1c、mALB检测的临床意义

Clinical significance of the level of CRP and HbA1c, mALB in patients with diabetic nephropathy

目的探讨糖尿病肾病患者血清C反应蛋白(CRP)、糖化血红蛋白(HbA1c)、尿微量蛋白(mALB)检测的临床意义。方法将2009年11月～2011年1月我院确诊的68例糖尿病患者作为观察对象,其中发生糖尿病肾病38例,其余30例为非DN患者,另选择同期32例健康体检者为正常对照组,分别检测各组血清CRP、HbA1c、mALB水平。结果 DN组、非DN组的CRP、HbA1c、mALB均分别明显高于正常对照组,差异有高度统计学意义(P<0.01);DN组的CRP、HbA1c、mALB均明显高于非DN组,差异有统计学意义(P<0.05)。经Spearman等级相关分析显示,血清CRP含量与mALB含量呈正相关,血清HbA1c含量与mALB含量也呈正相关(P<0.05)。结论通过检测C反应蛋白、HbAlc、mALB水平可以早期诊断糖尿病、监测糖尿病肾病的发生和发展,对延缓糖尿病肾病的发展具有重要的临床意义。

Objective To detect the level CRP and HbAlc, mALB in patients with diabetic nephropathy. Methods Sixty- eight cases diabetic patients diagnosed DN in our hospital from November 2009 to January 2011 as the observation object, including diabetic nephropathy of 38 cases, the remaining 30 cases of non-DN patients, and the other selected the same period 32 cases of healthy persons as normal controls group,and the serum levels of CRP, HbAlc, mALB were measured. Results The level of CRP, HbA lc, mALB in DN group, non-DN group were significantly higher than that of the normal control group, the difference had highly statistically significant （P 〈 0.01 ）; CRP,HhAlc, mALB in the DN group were sig- nificantly higher than non-DN group,the difference had statistically significant （P 〈 0.05）. Spearman rank correlation analysis showed that serum CRP levels and mALB content was positively correlated with serum HhAlc content and mALB of content, also showed a positive correlation （P 〈 0.05）. Conclusion The detection of C reactive protein, HbAlc, mALB level of early diagnosis of diabetes, monitoring of diabetic nephropathy and development and delay the development of dia- betic nephropathy has important clinical significance.
