动态磁敏感对比增强磁共振成像在脑肿瘤术前评估中的应用

摘要 目的研究患者的脑肿瘤的DSC图像及相关灌注指标对脑肿瘤的术前分级评估及肿瘤鉴别诊断的应用价值及意义。方法收集我科38例脑肿瘤患者的临床资料,患者入院后均行常规磁共振及动态磁敏感对比增强检查,测量瘤实质区、瘤旁1 cm及瘤旁2 cm的灌注指标。结果在肿瘤实性病灶区域,低级别胶质瘤组的灌注指标与其他组比较差异均有统计学意义(P<0.05);在测量区域的瘤旁1 cm区,高级别胶质瘤组的灌注指标与其他组比较差异有统计学意义(P<0.05),四组的脑肿瘤实性病灶区与测量区的瘤旁1 cm、瘤旁2 cm区域的灌注指标比较,差异均有统计学意义(P<0.05)。结论动态磁敏感对比增强技术在脑肿瘤术前病理学级别评估及鉴别诊断方面具有重要价值,值得临床推广应用。 Objective To study the application value and significance of DSC images and associated perfusion index for preoperative grading and tumor differential diagnosis in patients with brain tumor. Methods Collected 38 cases of brain tu- mor patients with the clinical data, patients admitted after underwent conventional magnetic resonance and dynamic sus- ceptibility contrast enhanced examination, measurement of tumor parenchyma, beside tumor and tumor adjacent to 2 cm perfusion index. Results In tumor solid lesion area, low grade gliomas group perfusion index and other group compare dif- ference to all had statistical significance; In the area of measurement of peritumoral zone 1 cm, high grade gliomas group perfusion index and other groups had a significant difference, four groups of brain tumor solid lesion area and measurement area beside tumor, tumor adjacent to 1 cm, 2 em LCM regional perfusion index were compared, the differences were statis- tically significant. Conclusion Dynamic susceptibility contrast enhancement technique has great value in brain tumor preop- erative histopathological grade assessment and differential diagnosis and it is worthy of clinical application.
