厄贝沙坦治疗原发性高血压伴左心室肥厚疗效的临床研究

Clinical study on the curative effect of the irbesartan treatment for essential hypertension its left ventricular hypertrophy

目的研究厄贝沙坦治疗杓形和非杓形原发性高血压伴左心室肥厚降压效果及对左心室重构的影响。方法选择符合诊断标准的1～2级原发性高血压伴左心室肥厚患者141例,分为杓形组104例和非杓形组37例,均用厄贝沙坦(150 mg,qd)治疗6个月,观察两组治疗前后血压、左心室重构指标(LVMW、EDV、ESV、EF)的变化。结果两组降压效果有统计学意义(P<0.05),杓形组优于非杓形组(P<0.05);杓形组有明显左室重构作用,而非杓形组无左室重构作用(P>0.05)。结论厄贝沙坦治疗原发性高血压降压效果显著,对杓形原发性高血压可达到左心室重构效果,但对非杓形原发性高血压降压效果比杓形差,左心室重构效果不明显。

Objective To study the clinical curative effect of the Irbesartan treatment for essential hypertension with left ventricular hypertrophy. Methods All of 141 cases of 1-2 grade essential hypertension with left ventricular hypertrophy in accordance with the criteria were chosen for the diagnosis. There were 104 cases in the dipper group and there were 37 cases in the non-dipper group. The two groups were both treated with irbesartan （150 mg, QD） for 6 months. The blood pres- sure changes and left ventricular remodeling index （LVMW, EDV, ESV, EF） changes were observed before and after treat- ment. Results The antihypertensive effect in the two groups was significant different （P 〈 0.05）, and the effect of the dipper group was better than that of the non-dipper group （P 〈 0.05）; the dipper group had significant left ventricular remodeling, and the non-dipper group had no left ventricular remodeling （P 〉 0.05）. Conclusion The antihypertensive effect of the Irbesar- tan treatment for essential hypertension is significant and Irhesartan has the left ventrieular remodeling effect on the dipper essential hypertension; but the antihypertensive effect of the Irbesartan treatment for the non-dipper essential hypertension is inferior to that of the Irbesartan treatment for the dipper essential hypertension and the effect of left ventricular remodel- ing on the Irbesartan treatment for the non-dipper essential hypertension is not obvious.
