鸦胆子油乳剂治疗恶性胸腔积液疗效观察

Emulsion of Brucea Javanica oil in treatment of malignant pleural effusion

目的观察鸦胆子油乳剂胸腔内注入治疗恶性胸腔积液的疗效及不良反应。方法 经病理或细胞学明确诊断为恶性肿瘤合并恶性胸腔积液患者78例,随机分为治疗组(40例),对照组(38例)。行胸腔置入中心静脉导管闭式引流术引流胸腔积液后,治疗组胸腔内注入鸦胆子油乳100 mL/次;对照组胸腔内注入博来霉素45mg/次,比较两组疗效及不良反应。结果 治疗组总有效率(75.0%)与对照组(76.3%)相近(P>0.05);治疗组不良反应(15.0%)轻于对照组(63.2%)(P<0.05)。结论 鸦胆子油乳胸腔内注入治疗恶性胸腔积液疗效满意,不良反应轻。

Objective To investigate the efficacy and safety of emulsion of Brucea Javanica oil （EBO） in treatment of malignant pleural effusion. Methods Seventy-eight patients with malignant pleural effusion were enrolled in the study. After closed drainage of pleural effusion with thoracic central venous catheter, patients were randomly assigned to receive intrapleural EBO 100 mL （EBO group, n = 40） or intrapleural bleomycin 45 mg （ control group, n -- 38 ）, respectively. The efficacy and adverse reactions were compared between two groups. Results There was no significant difference in effective rate between two groups （75.0% vs 76. 3% , P 〉0. 05） ; however, the adverse reaction was less severe in EBO group than that in control group （ 15.0% vs 63.2% ,P 〈0.05）. Conclusion Intrapleural emulsion of Brucea Javanica oil is effective and has less adverse reactions in treatment of malignant pleural effusion.
