复方甘草甜素、他克莫司及雅漾等治疗面部激素依赖性皮炎40例疗效观察

Clinical Effects of Compound Glycyrrhizin Injection,Tacrolimus Ointment,Avene on Facial Corticosteroid Addictive Dermatitis

目的观察序贯使用复方甘草甜素、他克莫司软膏、雅漾修护保湿霜配合雅漾活泉水冷喷治疗面部激素依赖性皮炎短期和长期疗效。方法将82例患者随机分为两组。对照组予复方甘草甜素注射液40mL静滴,1次/d,共2周;停用后外用0.03%他克莫司软膏继续治疗2周。治疗组在对照组基础上早晚使用雅漾活泉水冷喷。4周药物治疗后治疗组早晚使用雅漾活泉水冷喷基础上外涂雅漾修护保湿霜,对照组早晚使用维E霜,继续观察20周。对比两组患者短期和长期疗效。结果治疗2周后,治疗组红斑明显减轻28例(70.0%),水肿明显减轻30例(75.0%),瘙痒明显改善29例(72.5%);对照组分别为19例(46.3%),21例(51.2%)和19例(46.3%)。两组比较差异均有统计学意义(P<0.05)。治疗8周内治疗组和对照组疗效差异无统计学意义(P>0.05),12周、24周时,治疗组疗效均优于对照组(P<0.05,P<0.01)。结论序贯使用复方甘草甜素、他克莫司软膏、雅漾修护保湿霜配合雅漾活泉水冷喷治疗面部激素依赖性皮炎能够很快改善患者对激素的戒断反应,短时间达到很好的治疗效果,同时提高患者皮肤的长期稳定性。

Objective To observe the short-term and long-term clinical effects of using Compound Glycyrrhizin Injec- tion （CGI）, tacrolimus ointment, Avene thermal spring water （ATSW） and Avene skin recovery cream （ASRC） sequentially in the treatment of facial corticosteroid addictive dermatitis （FCAD）. Methods Eighty-two patients with FCAD were divided into treatment and control groups. The patients in the control group were injected with CG140 mL, once a day for 2 weeks, and then were smeared with O. 03% tacrolimus ointment, twice a day for 2 weeks. The treatment group was added ATSW based on the control group, twice a day. The courses of medical therapy were g weeks. After 4 weeks, the treatment group was changed to use ATSW and ASRC, in the same time, the control group used Vitamin E Cream. Results After the patients were treated 2 weeks, the symptoms or signs such as erythema, edema, itching were significantly relieved. Treatment group＇ s effective rate of the erythema was 70.0% （28 cases）, edema 75.0% （30 cases）, itch- ing 72.5% （29 cases）. Control group＇ s effective rate of the erythema was 46.3% （19 cases） , edema 51.2% （21 cases）, itching 46.3% （19 cases）. There were significant differences between the two groups （ P 〈 0.05 ）. After the patients were treated 4, 8, 12, 24 weeks, there was no significant difference in two groups within 8 weeks （ P 〉 0.05 ）. There were significant differences compared with the control group within 12 and 24 weeks （ P 〈 0. 05, P 〈 0.01 ）. Conclusion Using CGI, tacrolimus ointment, ATSW and ASRC sequentially to cure FCAD can reduce abstinence reaction. The way can get a better treatment outcome in a short time, and enhance the permanent stability of treatments.
