阿立哌唑对长期住院男性精神分裂症患者临床疗效和生活质量的影响

Influences of aripiprazole on curative effect and life quality of long-term hospitalized male schizophrenics

目的 探讨阿立哌唑对长期住院男性精神分裂症患者临床疗效和生活质量的影响,为临床干预提供依据.方法 将76例长期住院男性精神分裂症患者随机分为两组,研究组37例,口服阿立哌唑治疗,对照组39例,口服氯氮平治疗,观察12周.于治疗前及治疗12周末采用简明精神病量表评定临床疗效,副反应量表评定不良反应;治疗后采用生活质量综合评定问卷评定生活质量.结果 治疗后两组简明精神病量表评分均较治疗前有显著下降（P＜0.01）;治疗12周末,研究组显效率81.1%、有效率89.2%,对照组分别为71.8%、84.6%,两组显效率、有效率比较差异无显著性（P＞0.05）.治疗后研究组生活质量综合评定问卷各维度评分均显著高于对照组（P＜0.01）.研究组不良反应主要表现为静坐不能、失眠等,对照组主要表现为便秘、体质量增加、心电图异常、头昏等.结论 阿立哌唑治疗长期住院精神分裂症疗效显著,总体疗效与氯氮平相当,不良反应轻微,安全性更高,改善患者的生活质量优于氯氮平.

Objective To explore the influences of aripiprazole on curative effect and life quality of longterm hospitalized male schizophrenics in order to provide basis for clinical intervention. Methods Sevenly- six long-term hospitalized male schizophrenics were randomly assigned to research group （n=37） taking orally aripiprazole and control group （n=39） did clozapine for 12 weeks. Efficacies were assessed with the Brief Psychiatric Rating Scale（BPRS） and adverse reactions with the Treatment Emergent Symptom Scale （TESS） before treatment and at the end of the 12^th week; qualities of life were assessed with the Generic Quality of Life Inventory-74 （GQOLI-74）. Results After treatment the BPRS scores of both groups low- ered more significantly compared with pretreatment （P〈0.01） ; at the end of the 12^th week, obvious effective and effective rate were respectively 81.1% and 89.2% in research and 71.8% and 84.6% in control group, which showed no significant differences （P〈0.05）. All dimensions＇ scores of the GQOLI-74 were significantly higher in research than control group after treatment （P〈0.01）. Adverse reactions were mainly akathisia, insomnia etc. in research and constipation, weight gain, abnormal ECG, dizziness etc. in control group. Conclusion Aripiprazole has an evident effect on long-term hospitalized schizophrenics, its total efficacy is equivalent to clozapine and it has mild adverse reactions, higher safety and an advantage in improving patients＇ quality of life over clozapine.
