双功彩色多普勒血流影像和^(99m)Tc-MDP对乳腺良恶性肿瘤鉴别诊断价值

Evaluation of differential diagnosis of duplex Color Doppler and 99m Tc MDP in breast tumors

目的 采用双功彩色多普勒血流显像和99mTc MDP乳房显像相结合评价乳腺良恶性肿瘤的鉴别诊断价值。方法 用CDFI检查 1 5 2例 1 5 8个乳腺肿块 ,定量分析彩色像素平均密度(MCPD)和血管数目 ,用PD取样计算Vmax、Vmin、Vmean及RI值 ;用99mTc MDP检测 32例乳腺肿块 ,计算对乳腺癌诊断的敏感性、特异性、准确性。结果 提示彩色像素平均密度 >0 0 6 1± 0 0 0 1 7,峰值血流速度 >2 3 5 5cm/s± 1 2 3,RI >0 71± 0 0 1 1 ,血管数目 >1 6 7± 0 0 6 5 ,99mTc MDP乳房显像阳性 ,考虑为乳腺癌 ,其敏感性 88%、特异性 82 %、准确性 89% ;99mTc MDP的敏感性 90 %、特异性80 %、准确性 87%。结论 CDFI、PD和 99mTc MDP联合运用对乳腺良恶性肿瘤的鉴别诊断有较高的临床价值。

Objective To evaluate the significance of the differential diagnosis of duplex Color Doppler imaging and 99m Tc MDP in breast tumors. Methods In 152 patients,158 masses of breast were investigated with duplex Color Doppler imaging and PD.MCPD,PSV,TAV,RI and vessel numbers were analyzed quantitatively.Of the 158 masses,32 were studied with 99m Tc MDP.The diagnostic sensitivity,specificity and accuracy of CDFI and 99m Tc MDP for breast tumor were calculated. Result It was shown that when MCPD was over 0 061,PSV over 23,RI over 0 70,vessel number over 1 6, 99m Tc MDP was positive,the possibility of breast cancer should be considered.Diagnostic sensitivity,specificity and accuracy of CDFI and 99m Tc MDP for breast tumor were 88%,82%,89% and 90%,80%,87% respectively. Conclusion CDFI conjugated with 99m Tc MDP may increase the accuracy in differentiation of malignant and the benign of breast tumors.
