天津市医疗机构病床利用效率分析

Study on the utilization efficiency of health resources in Tianjin

目的:了解2009年天津市医疗机构病床利用效率情况。方法:比较分析天津市与全国平均水平及其他直辖市的卫生资源总量利用效率,秩和比分档分析该市不同类型卫生机构、不同科别专科医院及隶属于不同行政管理部门的二级及以上医疗机构的床位利用效率。结果:社区卫生服务中心床位利用效率过低,综合医院及乡镇卫生院较高;肿瘤、胸科医院床位利用效率过高,口腔、精神病及康复医院利用率不高;市内六区卫生局直属医院床位利用效率过低,医科大学直属医院效率过高、存在安全隐患,二区三县直属医院效率最优。结论:政府应调整资源配置方向,加大对社区卫生服务的投入,调整医疗机构病床配置,实现卫生资源的优化配置与利用。

Objective： To get knowledge of the utilization efficiency of total health resources of Tianjin in 2009. Methods： The total health resources of Tianjin, the national average and other municipalities were compared and analyzed. The bed utilization efficiency via Rank-Sum Ratio in different types of health organizations, different divisions of specialist hospitals, and secondary and above medical institutions belonging to different administrative de- partments were analyzed. Results： The bed utilization efficiency was too low in community health service centers, and high in general hospitals and township health centers. It was too high in the hospitals specializing in tumor and chest diseases, and not so high in the hospitals specializing in dental, mental and recovery. It was too low in the hos- pitals affiliated to the health bureau in the urban areas , and too high in the hospitals affiliated to the medical universi- ties which had a potential danger, and was the best in the hospitals within two areas and three counties. Conclusion： The govemment should adjust the direction of resource allocation, increase the investment in community health serv- ices and regulate the hospital bed allocation in medical institutions to achieve at the optimized allocation and efficient utilization of health resources.
