乳腺癌p53基因表达与辅助化疗的关系

p53 Gene Expression and Perioperative Chemotherapy in Breast Cancer

目的 探讨乳腺癌ｐ５３ 基因表达与辅助化疗的近期临床疗效的关系。方法 采用ＳＰ 法对１０８ 例乳腺癌做免疫组化染色，观察各型乳腺癌中ｐ５３ 基因的表达及与术前化疗疗效的关系，并进行临床相关因素分析。术前化疗采用ＣＭＦ 方案（ＣＴＸ６００ ｍ ｇ／ ｍ ２，静脉推注，第１ 天和第８ 天；ＭＴＸ ３０ ｍ ｇ／ ｍ ２，静脉推注，第１ 天和第８ 天；５Ｆｕ ５００ ｍ ｇ／ ｍ ２ 静脉滴注，第１ 天和第８ 天）１ 个疗程化疗后，进行手术。结果 术前化疗后近期临床疗效与ｐ５３ 基因表达呈负相关，ｐ５３ 基因表达与乳腺癌病理类型、激素受体相关，与肿瘤大小、淋巴结转移无关。结论 乳腺癌ｐ５３ 基因表达状态对辅助化疗有重要的参考价值，并能判断患者的预后。

Objective To investigate the relationship between p53 gene expression and the short time effect of perioperative chemotherapy in breast cancer patients.Methods 108 cases of breast cancer were studied the p53 gene expression was detected by immunohistochemical staining(S P method) and the shorttime effect of perioperative chemotherapy,clinical and pathological parameter were analyzed.Before operation,the patients accepted one cycle of CMF chemotherapy(cyclophosphamide,600 mg/m 2 i.v. d1 and d8;methotrexate,30 mg/m 2 i.v. d1 and d8;5 Fluorouracil,500 mg/m 2 i.v. d1 and d8).Results The shorttime effect of perioperative chemotherapy was in negative correlation to p53 gene expression.The pathological type and the levels of ER and PR were related to p53 gene expression,while the TNM stage,axillary lymphonode metastasis were not related to it.Conclusion This study indicates that the detection of p53 gene expression has important value to instruct breast cancer chemotherapy.
