摘要 [目的]探讨腹腔镜胃癌根治术后早期应用谷氨酰胺强化的肠内营养(GLN)的作用。[方法]将96例腹腔镜胃癌根治术后病人随机分为谷氨酰胺强化肠内营养组(GLN组)和常规肠内营养组(EN组)。比较两组病人术前1d、术后8d营养状况、免疫功能以及术后肛门排气时间、住院天数和费用。[结果]GLN组术后第8天清蛋白、前清蛋白、转铁蛋白、IgA、IgG、CD4、CD8较EN组明显增高,术后肛门排气和住院时间缩短,住院费用降低(P<0.05)。[结论]腹腔镜胃癌根治术后病人早期应用GLN有利于改善病人术后的营养状况和机体免疫功能,促进胃肠道功能恢复,节约医疗费用。 Objective: To probe into the effect of early application of glutamine -enriched enteral nutrition for patients with gastric carcinoma after laparoscopic gastrectomy. Methods:A total of 96 patients after laparoscopic gastrectomy were randomly divided into glutamine - enriched enteral nutrition (GLN) group and routine enteral nutrition (EN) group. To compare the nutritional status and immune function, the times of anal exsufflation, hospitalization days and the price of medical treatment of the patients at 1 day before operation and 8 days after operation, between the two groups. Results: The level of albumin, prealbumin, transferrin, IgA, IgG, CD4, CD8 of GLN group patients were significantly higher thant that of EN group at the postoperative 8th day,the time of anal exsufflation,the hospital days and Hospitalization costs were significantly lower in GLN group than those in the EN group(P〈0.05). Conclusion : Early application of glutamine - enriched enteral nutrition for patients with gastric carcinoma after laparoscopic gastrectomy not only could improve patients' postoperative nutritional status and immune function,but also promote patients' recovery of gastrointestinal function, reduce medical care costs.
