妊娠糖尿病孕妇糖化血红蛋白与甲状腺激素水平分析

Analysis of Glycosylated Hemoglobin and Thyroid Hormones Levels in Patients with Gestational Diabetes Mellitus

目的:探讨妊娠糖尿病孕妇糖化血红蛋白(HbA1c)、甲状腺激素水平的变化及临床意义。方法:选择妊娠糖尿病孕妇127例为病例,随机选取同期正常妊娠妇女179例,测定三碘甲状腺原氨酸(T3)、游离三碘甲状腺原氨酸(FT3)、总甲状腺素(T4)、游离甲状腺素(FT4)、促甲状腺素(TSH)及空腹血糖(FBG)水平,并与HbA1c进行相关性分析。结果:与正常妊娠妇女相比,妊娠糖尿病孕妇的T3、FT3、T4、FT4水平明显下降,差异均有统计学意义(P<0.01);HbA1c与FBG明显上升(P<0.01);HbA1c与FBG呈正相关(r=0.44,P<0.01),与FT3呈负相关(r=-0.29,P<0.05)。结论:HbA1c是监测妊娠糖尿病病情的良好指标,妊娠糖尿病孕妇存在甲状腺激素水平低下的现象。

Objective： To investigate the variations of glycosylated hemoglobin （ HbA1 c ） and thyroid hormones and their clinical significance in gestational diabetes mellitus patients. Method： A number of 127 gestational diabetes mellitus patients were selected as case group, 179 normal pregnant women in the same period were randomly selected as control group. Triiodothyronine （T3） , thyroid hormones free triiodothyro-nine （ FT3 ）, total thyroxine （ T4 ）, free thyroxine （ FT4 ）., thyroid stimulating hormone （ TSH ） and fasting blood sugar （FBG） were detected and correlation analysis were conducted between HbAlc and the above in-dex. Result： The T3 ,FT3 ,T4 ,FT4 levels were significantly lower （ P〈0. 05 ） in gestational diabetes mellitus patients, while HbAlc and FBG were significantly higher （ P〈0.01 ） than those in normal pregnant women. HbAlc had a positive correlation with FBG （ r=0.44, P〈0.01 ） , and a negative correlation with FT3 （ r=-0.29,P〈0.05 ）. Conclusion： HbAlc is a good index for monitoring the gestational diabetes condition, low levels of thyroid hormone are existed in diabetes mellitus patients.
