多西他赛联合顺铂不同化疗途径治疗宫颈癌的临床疗效对比观察

Comparative Study of Docetaxel Combined with Cisplatin Chemotherapy in Treating Cervical Cancer by Different Approaches

目的:对多西他赛联合顺铂经过不同的化疗途径治疗宫颈癌的临床效果进行对比研究,为临床治疗宫颈癌提供理论和依据。方法:从我院随机选取2009年2月至2011年2月期间32例符合新辅助化疗的患者,将其随机分成治疗组和对照组,治疗组14例,对照组18例,两组均给予多西他赛联合顺铂化疗,对照组采取静脉化疗,治疗组在DSA影像指导下行动脉灌注化疗,两种同时采用止吐治疗及静脉水化3d,治疗2-3疗程后3周左右行宫颈癌根治术,观察两组化疗副反应、化疗后肿瘤缓解情况、手术时间及化疗手术总费用等。结果:通过对比试验发现,对照组的手术时间和化疗费用要显著低于治疗组,化疗后肿瘤缓解情况比较,治疗组92.9%稍好于对照组的88.9%,但无显著差异(P>0.05),治疗组的不良反应率要显著少于对照组,具有统计学意义(P<0.05)。结论:通过试验我们认为多西他赛联合顺铂化疗具有较好的缓解肿瘤的效果,在不同途径中,静脉化疗的操作简便、经济,但副反应较多,动脉化疗较复杂,缓解率稍好于静脉治疗,有条件的医院可选择动脉化疗。

Objective： Comparative study on the clinical otherapy approaches with docetaxel and cisplatin, in order to efficiency of cervical cancer by different chem-provide theory and evidence for clinical treat-ment to cervical cancer. Method： 32 cases were randomly selected from our hospital from Feb. 2009 to Feb. 2011 with neoadjuvant chemotherapy, divided into treatment group and control group. The treatment group included 14 cases, the control group included 18 cases. Both groups got chemotherapy with docetaxel and cisplatin. The control group took venous chemotherapy, the treatment group in DSA image guidance downside artery perfusion chemotherapy, both group use stop spitting treatment and intravenous hydration at 3 days, Radical hysterectomy Was used about three weeks after the treatment of 2-3 courses. To observe side effects of chemotherapy tumor ease, operation time and chemotherapy surgery total cost, etc. Result： Through the contrast test, we found that the operation time and chemotherapy cost of control group was signif-icantly lower in treatment group. Chemotherapy after tumor ease the situation of the treatment group was 92. 9%, better than control group＇ s 88.9% , but no significant difference （P〉0.05 ）. The adverse reaction rate of treatment group was significantly less than control group, which had statistically significant （P〈0.05 ）. Conclusion： Docetaxel combined with cisplatin chemotherapy has good effect of alleviate tumor. In different ways, venous chemotherapy is easy operation and economy, but more side effects, and arterial chemotherapy is more complex, ease slightly better rate in vein treatment. Some rich hospital can choose arterial chemo-therapy to reduce adverse reaction.
