上消化道穿孔145例诊疗体会

Clinical experience of the treatment of upper digestive tract perforation

目的:总结上消化道穿孔的诊疗经验。方法:对145例上消化道穿孔病例的临床资料进行回顾性分析。结果:溃疡穿孔126例,恶性肿瘤穿孔15例,其他病因引起的穿孔4例。其中135例进行手术治疗,均治愈出院;10例行保守治疗,治愈7例,死亡2例,转外院1例。结论:上消化道穿孔病情急,变化快,一旦确诊应尽早手术治疗,合理选择术式,有利于挽救生命及减少并发症的发生。

To sum up the clinical experience of the treatment of upper digestive tract perforation. Methods： Retro-spectively analyse the clinical charts of 145 cases of upper digestive tract perforation. Results： The causes of upper digestive tract per- foration included peptic ulcer （ 126 cases ）, gastrointestinal cancer （ 15 cases） and others （4 cases）. Patients with surgical interven- tion were all cured and discharged （135 cases）. Ten patients received conservative treatment and 7 of them were cured, 2 of them died, and 1 patient was transferred to another hospital. Con^lutton： As gastrointestinal emergency , upper digestive tract perforation can progress rapidly. Once diagnosed with upper digestive tract perforation , patients should receive surgical intervention immediatelyto be saved and also to lower the incidence of complications.
