卧床便秘患者实施护理干预后生活质量效果观察

Effect of nursing intervention on life quality in constipation patients lying in bed

目的了解住院卧床便秘患者实施护理干预后生活质量的情况，为开展提壶开底推拿护理干预提供有力依据。方法采用问卷调查方式对98例卧床便秘患者进行观察，采取手指点按云门穴和中府穴、转揉腹部经络及按摩肛管治疗便秘后，对使用Bristol大便性状量表及便秘患者症状自评量表评分的改变，健康调查量表（SF-36）干预前后评分的改变，评估推拿护理及心理护理干预对住院卧床便秘患者生活质量的影响。结果通过对卧床便秘患者实施提壶开底推拿护理干预后，患者在大便的性质、腹痛、腹胀、排便费力、排便疼痛等方面得到明显的改善；通过健康调查量表干预前后便秘患者在生理职能、躯体疼痛、情感职能、精神健康、社会功能和总体健康等8个维度评分明显增加，t=69．5—77．2。结论运用提壶开底推拿手法护理干预便秘患者，能恢复患者大肠正常的蠕动功能，增强胃肠的消化吸收能力，明显提高了生活质量，对住院卧床患者便秘问题的近期作用与远期疗效有广泛的临床意义。

Objective To learn the life quality in constipation patients lying in bed after they received nursing intervention and to provide powerful support for nursing intervention with taking mention pot of open bottom manipulation. Methods 98 constipation patients were surveyed by questionnaires ; Yunmen and Zhongfu were pressed by fingers; constipation were treated by kneading abdominal channels and collaterals in circles and massaging anal tube. Bristol solid waste quality scale, symptom self-estimation scale, and health survey scales （ SF-36 ）were used before and after nursing to estimate the effects of manipulation nursing and psychological nursing intervention on the life quality in constipation patients lying in bed. Results After taking mention pot of open bottom manipulation nursing intervention on the consti- pation patients lying in bed, it obviously improve the nature of patients＇ solid wastes, abdominal pain, abdominal distension, defecation disability and defecation pain. By health survey scale before and after intervention, patient scores of 8 dimension including physiological function, body pain, emotional function, mental health, social function and general health were obviously increased （ t =69.5 - 77.2 ）. Conclusions Application of manipulation on nursing constipation patients can restore normal peristalsis function of the large intestine, enhance the digestive absorption function of the stomach and intestine, and improve life quality obviously. It has a wide clinical significance for the short-and long-term curative effect on constipation in patients lying in bed.
