椎间盘退行性变的生物学治疗进展 被引量:10

摘要 椎间盘退行性变可诱发脊柱退行性改变、椎间盘髓核突出、椎管狭窄等疾病,是引起下腰痛的主要原因,因其具有高发病率和高致残率的特点,给社会和家庭带来了沉重的负担。目前的治疗措施主要包括髓核摘除、椎管减压及退变脊柱节段融合等,但只能在短期内缓解临床症状,其远期疗效并不显著,且存在并发症多的缺点^[1]。近年来,随着分子生物学、细胞生物学的深入研究,发现椎间盘细胞数量减少、压力损伤终板的微骨折、细胞外基质降解增加、炎症因子的异常表达等均可导致椎间盘的主要成分代谢失衡,从而引发椎间盘退行性变的发生、发展。研究人员以此为基础,针对退行性变的不同阶段,利用基因工程、细胞工程、组织工程手段来恢复退行性变椎间盘细胞数量或重建椎间盘解剖结构和生理功能,从根本上为治疗椎间盘退行性变提供了有效策略。
