静脉腔内激光治疗联合手术治疗下肢静脉曲张并发症分析

Analysis on complications following endovenous laser treatment combined with surgery for the treatment of varicose veins of lower extremety

目的探讨静脉腔内激光治疗（endovenouslasertreatment，EVLT）联合手术治疗下肢静脉曲张术后并发症的原因、预防和治疗。方法回顾性分析1325例（共1725条患肢）EVLT联合手术治疗下肢静脉曲张患者的临床资料。结果发生术后并发症共274例（20．68％），其中皮下淤血82例（6．19％），隐神经损伤58例（4．38％），皮肤灼伤36例（2．72％），皮下条索状硬结、硬块、红肿及严重疼痛60例（4．53％），下肢血栓性浅静脉炎12例（0．91％），下肢深静脉血栓形成2例（0．15％），足踝部水肿43例（3．25％）。结论EVIT联合手术治疗下肢静脉曲张具有安全、有效、微创、操作简便等优势，但应重视对其并发症的预防和治疗。

Objective To investigate the reasons for complications following endovenous laser treatment （EVLT） combined with surgery for the treatment of varicose veins of lower extremety and to disscuss the prevention and treatmant of the complications. Methods The clinical data of 1 325 cases （ 1 725 affected legs） undergoing EVLT combined with surgery were analyzed retrospectively. Results There were total of 274 cases （20.68%） of complications, including 82 cases （6.19%） of subcutaneous ecchymosis, 58 cases （4.38%） of sapheuous nerve injury, 36 cases （2.72%） of skin bums, 60 cases （4.53%） of subcutaneous strip seleroma, lump, swelling and severe pain, 12 cases （0.91%） of su- perfacial thrombophlebitis of the lower extremity, 2 cases （0.15%） of deep vein thrombosis of the lower extremity, 43 cases （3.25 % ） of foot and ankle edema. Conclusion EVLT combined with surgery for the treatment of varicose veins of lower extremety has the advantages of safety, effectiveness, minimal invasion, and simple operation. However, empha- sis should be placed on the prevention and treatment of complications.
