彩色多普勒超声对儿童肠系膜淋巴结炎的诊断价值

Value of color Doppler ultrasound in diagnosis of mesenteric lymphadenitis in children

目的探讨彩色多普勒超声对儿童肠系膜淋巴结炎的诊断价值。方法回顾分析243例肠系膜淋巴结炎患儿(患儿组)和140例正常儿童(对照组)的超声表现,对肠系膜淋巴结炎患儿治疗后的情况进行随访。结果患儿组肿大淋巴结位于右中下腹部肠系膜处1589个,脐周部1430个,左侧腹部69个,彩色多普勒血流成像可探及较丰富的淋巴门血供;对照组淋巴结位于右中下腹部肠系膜处530个,脐周部274个,左侧腹部42个,彩色血流信号不丰富,呈点状或小线样低速血流,两组比较差异有统计学意义(P﹤0.05)。抗炎治疗1周后复查,患儿组肿大淋巴结均有缩小或消失。结论彩色多普勒超声能清晰显示肿大的肠系膜淋巴结,对临床明确诊断和观察疗效有一定应用价值。

Objective To explore the value of color Doppler ultrasound in diagnosis of mesenteric lymphadenitis in children.Methods Two hundred and forty-three children with mesenteric lymphadenitis （children group） and 70 cases of health children （contrast group） were enrolled in this study, their uhrasonic appearance were analyzed retrospectively and children group were followed up after treatment.Results There were 1589 mesenteric lymph nodes in the right lower abdomen, 1430 lymph nodes in the peripheral umbilicus, and 69 lymph nodes in the left abdomen.Plentiful blood signal was detected by color Doppler ultrasound in lymphatic door.While there were 530 mesenteric lymph nodes in the right lower abdomen, 274 lymph nodes in the peripheral umbilicus, and 42 lymph nodes in the left abdomen.Punctuate and line-like blood flow with low velocity was detected by color Doppler uhrasound,Therc was significant difference between two groups （P〈0.05）.After one-week treatment of anti-inflammatory, the enlarged lymph nodes were decreased or disappeared in children group.Conclusion Color Doppler ultrasound can show the enlarged mesenteric lymph nodes clearly, which has certain application value of clinical diagnosis and efficacy observation.
