9例股骨头骨折临床治疗与分析

The clinical treatment and analysis of 9 cases femoral head fractures

目的探讨不同类型股骨头骨折的治疗方法和并对其疗效进行分析。方法回顾分析广州中医药大学第一附属医院一骨科自2006至2011年收治的9例股骨头骨折患者，根据Pikpin分型，Ⅰ型2例，Ⅱ型3例，Ⅲ型2例，Ⅳ型2例，根据患者受伤的特点结合Pikpin分型，采用不同的手术治疗方法，并对其结果进行分析。结果9例经6个月～3年的随访，根据Thompson—Epstein临床和影像学评定标准，优4例，良3例，差2例，优良率77．8％。结论股骨头骨折是罕见的骨折，根据受伤特点结合Pikpin分型，采用不同的治疗方法，术后早期功能锻炼，可取得较满意的疗效。

ObJective To discuss different treatment strategies in fractures of the femoral head, and analyse the clinic effects.Methods 9 cases of femeral head fractures were treated in Traumatic orthopedic of the first hospital of Guangzhou University of TCM from 2006 to 2011. According to Pipkin classification, there were type I in 2 cases, type II in 3 cases, type III in 2 case, and type IV in 2 case. We used different operations according to the patient＇ s characteristics and Pipkin＇ s classification, and evaluated the outcomes. Result 9 cases were follow-up for 3 months to 3 years. Outcome was evaluated with the Thompson and Epstein score. In all 9 cases, excellent result was achieved in 4 cases, good in 3 cases, poor in 2 case,the excellent-good rate was 77.8%. Conclusion The femoral head fracture was rare fracture, according to the patient＇s characteristics and Pipkin＇s classification, we used different operations, and early functional exercise should be done after operation, and then we obtained good outcomes.
