背阔肌游离皮瓣在治疗下肢软组织缺损中的应用研究

Free flap in the treatment of lower limb soft tissue defect of the application

目的探讨游离皮瓣在治疗下肢伤软组织缺损中的应用价值。方法回顾性分析2008年1月～2012年1月我院采用游离皮瓣进行治疗的39例下肢软组织患者的临床资料,并以同期采用一期植皮治疗的38例患者为对照组,比较治疗效果。结果观察组治疗有效率为100.0%,并发症发生率为2.56%,对照组分别为97.37%和10.53%,组间比较差异具有统计学意义(χ2=35.636,58.948,P=0.000)。观察组手术时间为(124.5±35.7)min,住院时间为(12.2±4.7)d,治疗费用为(11456.6±1584.7)元,对照组分比别为(108.4±29.6)min、(18.4±5.6)d和(14652.7±2584.5)元,组间比较差异具有统计学意义(t=154.267,54.240,317.962;P=0.000)。结论游离皮瓣治疗下肢软组织缺损疗效确切,值得临床进一步研究推广。

Objective To discuss application value of free flap in the treatment of soft tissue defect in the lower extremity injuries. Methods Retrospective analysis from January 2008 to January 2012 with our free flap for treatment of patients with lower limb soft tissue 39 cases clinical material, and the same period with a period of treatment with skin grafts 38 patients with the control group, more therapeutic effect. Results The observation group therapy was 100.0%, complication was 2.56%, and the control group were 97.37% and 10.53% respectively, comparative differences between groups are statistically significant （the chi-square= 35.636, 58.948, P = 0.000）. The observation group operation time was （124.5±35.7）min, length of hospital stay was （12.2±4.7）d, treatment costs for （11456.6±1584.7）yuan, control components for than don＇t （108.4±29.6） rain, （18.4±5.6）d and （14652.7±2584.5） yuan, comparative differences between groups are statistically significant （t=154.267, 54.240, 317.962; P = 0.000）. Conclusion Free flap treatment lower limb soft tissue defect symptoms, and is worth popularizing clinical further.
