10mm取石钳联合宫腔镜电切术治疗子宫黏膜下肌瘤的临床研究

Clinical research of the large biopsy forceps combined hysteroscopic electrocision for submucosal myoma

目的探讨10mm取石钳联合宫腔镜电切术治疗子宫黏膜下肌瘤的临床应用价值。方法将2009年1月～2011年12月在该院使用10mm取石钳联合宫腔镜下电切术治疗的29例子宫黏膜下肌瘤作为联合治疗组,2006年1月～2008年12月在该院单纯采用宫腔镜电切术治疗的27例子宫黏膜下肌瘤作为对照组,两组病例在年龄、肌瘤大小、个数、类型等一般情况方面无差异,该文主要比较分析两组手术病例的一般情况、手术治疗效果、手术时间、出血量、膨宫液体用量、并发症等情况。结果与对照组相比联合治疗组病例的手术时间更短,膨宫液体使用量更少,子宫穿孔、周围脏器损伤、大出血,稀释性低钠血症、宫腔感染等并发症发生率也更低,两者之间的差异有显著性。结论 10mm取石钳联合宫腔镜电切术治疗子宫黏膜下肌瘤是一种操作简便、安全有效的治疗方法,值得临床推广应用。

[ Objective ] To evaluate the effect and the safety of the large biopsy forceps combined hysteroscopic electrocisiou for submucosal myoma. [ Methods ] In our study, 29 cases of submucosal myoma were performed by the large biopsy forceps combined hysteroscopic eleetrocision for submucosal myoma during Jan.2009 to Dec. 2011 （combination therapy group） and 27 cases of submucosal myoma were performed by simple hysteroscopic electroei- sion during Jan.2006 to Dec.2008 （control group） in the Sichuan Provincial Hospital for Women and Children. These cases of combination therapy group and control group have no difference in the age, the type, number and size of submueosal myoma. The effect of the treatment, the time of surgery, the loss of blood, uterine distending medium and complications of the groups were compared and analyzed. [Result] The combination therapy group had advantage on the time of surgery, the loss of blood, uterine distending medium and complications than the control group. [ Conclusion ] The treatment of the large biopsy forceps combined bysteroscopic eleetrocision for submucosal my- oma is effective and safety. The method is easy to learn and has extensive generalizing value in clinic.
