临床药师对脑血管病住院患者健康教育的工作模式探讨

Exploration of Working Mode of Health Education of Clinical Pharmacists in Hospitalized Patients with Cerebrovascular Disease

急性脑血管病发病率高、死亡率高、致残率高、复发率高、并发症多,为了使患者认识到脑血管病的重要性,临床药师拟定住院患者的健康教育流程,如采集病例、评估用药现况、制定个体化的教育方案及定期随访等。并总结经验教训,提出改进措施,为临床药师的工作提供了规范化的工作模式。

Acute cerebral vascular disease with high incidence, high mortality, high disability rate, high recurrence rate and many complications, in order to make patients aware of the importance of cerebrovascular, the clinical pharmacists drew out the health education process to the inpatients, such as the collection of cases, assessment of medication status, development of an individualized education program and regular follow-up, summed up the lessons, put forward the improvement measures, provided a standardized working mode for the clinical pharmacists.
