不同国产α-干扰素治疗慢性乙型肝炎临床对比观察

Clinical comparative study on α-interferon in treatment of chronic hepatitis B

目的对比观察不同的国产α-干扰素治疗慢性乙型肝炎的效果与不良反应。方法 150例符合纳入标准的慢性乙型肝炎患者被分为3组,每组50例。A组给予α-2a干扰素(安福隆)、B组给予α-2b干扰素(安达芬)和C组给予α-1b干扰素(赛若金)治疗,比较治疗前和治疗后3、6、和12个月的治疗效果和不良反应的发生率。结果 3组患者在治疗后12个月,ALT、AST和TBIL水平恢复正常,较治疗前差异显著(P<0.05);A、B和C组患者HBeAg转阴率均达到了48.0%,HBsAg转阴率分别为16.0%、8.0%和6.0%,HBV DNA转阴率分别为52.0%、52.0%和48.0%;B组患者发热、消瘦以及血小板一过性减少的并发症发生率明显较A组、C组高(P<0.05),而失眠抑郁的发生率B组比A组、C组要低(P<0.05)。结论常用国产α-干扰素治疗慢性乙型肝炎的疗效相似,其中安福隆效果尤为理想。

Objective To compare the effect and adverse reactions of various Chinese - made α- interferons in treatment of chronic hepa- titis B. Methods One hundred and fifty patients met with the inclusion criteria of chronic hepatitis B were divided into three groups ： patients in group A received IFN-ot2a （An Fulong）, patients in group B received IFN-α2b （An Dafen） and patients in group C were given with IFN -α1b（Sinogen）. The efficacy and adverse reactions were observed and compared between pre -treatment and 3, 6 and 12 months after treatment. Results After 12 months of treatment, serum levels of ALT, AST and TBIL in patients of these three groups were all returned to normal and the difference between pre - treatment and after α- interferon treatment was significant. The seroinversion rates of HBeAg in patients of groups A, B and C all reached 48.0%, and seroinversion rates of HBsAg in these 3 groups were 16.0% , 8.0% and 6.0% respectively, while HBV DNA neg- ative inversion rates in these 3 groups were 52.0% , 52.0% and 48.0% respectively. The incidence of fever, weight loss and transient decrease in amount of platelets in patients of group B were significantly higher than those of patients in groups A and C （ P 〈0.05 orP 〈0.01 ）, while the incidence of insomnia and depression in patients of group B was much less than those of patients in group A and group C （ P 〈 0.05, P 〈 0. 05 ）. Conclusion The efficacy of various domestic made α- interferons in treatment of patients with chronic hepatitis B was very similar, but the effi- cacy of An Fulong is particularly ideal.
