喉罩通气在喉癌手术中的应用

摘要 目的通过观察喉癌手术中喉罩通气对病人血流动力学和应激激素水平的影响,评价喉罩通气在喉癌手术中的应用价值。方法选择40例ASAⅠ-Ⅲ级的男性喉癌Ⅱ、Ⅲ期择期手术病人,按随机数字表法分为喉罩组(P组)和气管切开手术组(T组),每组20例。P组:快速麻醉诱导后置入双管型喉罩,固定后行机控呼吸,手术至颈部淋巴结清扫后行气管切开置入气管导管替代喉罩行控制呼吸后继续手术;T组:先在局部麻醉下行气管切开置入气管导管,固定后行机控呼吸。观察2组的MAP、HR、SpO2变化,观察时点为诱导前(P1/T1)、置入喉罩或气管切开置入气管导管即刻(P2/T2)、置入喉罩后或气管切开置入气管导管后10min(P3/T3)和置入喉罩后或气管切开置入气管导管后30min(P4/T4);观察麻醉前、置入喉罩后或气管切开置入气管导管后即刻、术毕和拔管后30min4个时点的血糖和血浆皮质醇水平。结果 T组病人在T2时点的MAP、HR明显高于T1时点(P<0.01),T3、T4时点与T1时点差异无统计学意义(P>0.05);T组与P组比较,T2时点的MAP、HR比P2时点明显升高(P<0.01)。T组自插管后各时间点的血糖均明显高于麻醉前(P<0.01),P组在术毕时点血糖上升与麻醉前比较差异有统计学意义(P<0.01);术毕和拔出导管后30min T组的血糖上升明显高于P组(P<0.05)。2组血皮质醇在术毕时点均高于麻醉前(P<0.05);拔罩或导管后30min的血皮质醇P组下降,而T组继续上升,T组与P组比较差异有统计学意义(P<0.05)。结论与局部麻醉下气管切开置入气管导管通气相比,双管型喉罩通气应用于喉癌切除术对患者血流动力学干扰小,引起的应激反应轻,同时能保证有效地通气。
