局部应用唑来膦酸对骨质疏松大鼠种植体骨结合的影响 被引量:6

摘要 目的探讨唑来膦酸系统治疗对骨质疏松(OP)时种植体骨结合的影响。方法 30只雌性SD大鼠随机分成三组:假手术组(A组)、骨质疏松组(B组)和唑来膦酸治疗组(C组)。B、C组行双侧卵巢切除术,A组接受假手术。术后12 w测量股骨骨密度(BMD),证实OP状态,并在大鼠左侧胫骨近中干骺端植入羟基磷灰石涂层种植体,A组、B组植入单纯HA涂层种植体,C组植入唑来膦酸钠-涂层种植体。种植术后第4、12周分两批处死动物,标本制作不脱钙切片,行形态学观察及骨计量学检测。结果种植术后4和12 w时,B组种植体骨结合率(IBCR)、新生骨量(NBV)均显著低于A组和C组(P<0.01);C组IBCR和NBV则与A组接近(P>0.01)。结论实验性OP可降低种植体骨结合率;局部应用唑来膦酸则可一定程度拮抗OP的负面影响,促进种植体愈合,使种植体骨结合率增加。 Objective To investigate the effect of local treatment with Zoledronate acid on osseointegration of dental implants in os- teoporotic rats. Methods 30 female rats were randomly divided into three groups: sham (A group), ovariectomized (B group) and zoledr- onate-treated (C group) groups. Animals in group B and C received bilateral ovariectomy, while in group A underwent sham-operation. 12 weeks after surgery, femoral bones of rats were examined for bone mineral density (BMD) to verify osteoporosis status. Then bone defects were made at the proximal metaphyses of tibiae and placement of hydroxyapatite (HA) coated titanium implants ( group A and B) or Zoledr- onate acide immobilized HA-implants ( group C) , were performed. Animals were killed at 4 and 12 weeks after implant surgery and under- calcified sections were prepared and examined histologically and histomorphometrically. Results At 4 and 12 week after implantation surgery, implant-bone contact rate (IBCR) and new bone volume (NBV) in group B were all significantly decreased compared to group A and C (P 〈 0. 01 ) ; while the bone indices ( IBCR, NBV) in group C were similar with those of group A (P 〉 0. 05 ). Conclusions Exper- imental osteoporosis can decrease 1BCR, while local application treatment with Zoledronate acid may resist the negative influence of osteopo- rosis and oromote bone graft healing and osseointagration of dental implants.
