PPH加中药坐浴治疗混合痔的疗效观察

PPH plus Sitz Bath with Traditional Chinese Drugs in the Treatment of Mixed Hemorrhoids：Observation of Therapeutic Effect

为探讨吻合器痔上黏膜环切钉合术（PPH）加中药坐浴治疗混合痔的临床疗效，采用PPH治疗以Ⅲ、Ⅳ期内痔为主的混合痔89例，术后采用中药坐浴。结果显示，本组手术均顺利完成，平均手术时间（15．2±6．7）min，平均住院时间（4．4±2．0）d。术后发生尿潴留8例；需用镇静药或止痛药11例；术后大便带血18例，其中2例再次手术止血，其余未经特殊处理，均自行好转；术后第3天肛门皮瓣水肿14例。随访1个月至2年，平均8．6个月，均无复发，无肛门狭窄发生。结果表明，PPH加中药坐浴治疗Ⅲ、Ⅳ期内痔为主的混合痔疗效肯定，且手术时间短，并发症少。

The objective of this study was to investigate the therapeutic effect of procedure for prolapse and hemorrhoids（PPH） plus sitz bath with traditional Chinese drugs in the treatment of mixed hemorrhoids. The 89 cases of mixed hemorrhoids（ Ⅲ , Ⅳ stage of internal hemorrhoids predominant mixed hemorrhoids） first received PPH, then received sitz bath with traditional Chinese drugs. As results, the operation per-formed on the 89 cases was smooth, average operative time was （15.2±6.7）min, average hospitalization time was （4.4±2.0）days;after operation uroschesis occured in 8 cases, 11 cases needed tranquilizer or ob- tundent,defecation with blood in 18 cases among them 2 cases received secondary operation to stop bleed-ing, rest cases were cured automatically without special management ;later 3 days following operation 14 ca-ses had edema of anal flap;the follow-up of 1-24 months（averaged 8.6 months） did not find recurrence and anal stenosis. Results show that PPH plus sitz bath with traditional Chinese drugs in the treatment of Ⅲ, Ⅳ stage internal hemorrhoids predominant mixed hemorrhoids has certain therapeutic effect, is of shorter operative time and less complications.
