Oculyzer和Sirius眼前节分析仪测量中央角膜厚度及形态的比较研究 被引量:1

摘要 目的比较Oculyzer和Sirius测量的中央角膜厚度(CCT)与A超测量CCT的差异,比较两种眼前节分析系统测量角膜前表面Q值的差异,为临床检查提供参考。方法选取近视屈光不正患者25例(50只眼),依次用Oculyzer和Sirius检查眼前节,再行A超测量CCT,比较Oculyzer和Sirius与A超测量的CCT的差异,比较Oc-ulyzer和Sirius测量的角膜最薄点的差异及前表面中央6 mm直径范围Q值差异。每种仪器测量5次取平均值,数据进行统计分析。结果 Oculyzer、Sirius、A超测量CCT均数分别为:(546.70±31.94)μm、(535.96±31.96)μm、(541.00±32.60)μm,Oculyzer测量值较A超厚,而Sirius测量值较A超薄,两两比较差异有统计学意义(P<0.05),高度相关,一致性好;Oculyzer和Sirius测量角膜最薄点的厚度均值分别为:(543.96±31.73)μm、(533.00±32.19)μm,两者比较差异有统计学意义(P<0.05),但相关性和一致性好。Oculyzer和Sirius测量角膜前表面中央6 mm直径范围Q值分别为:-0.1912±0.1134和-0.1202±0.1083,差异有统计学意义(P<0.05),无明显相关性,但一致性较好。结论 Oculyzer和Sirius测量CCT与A超比较虽有统计学差异,但有高度相关性和一致性,可取代A超。Oculyzer测量角膜最薄点厚度较Sirius测量值高,但有高度相关性和一致性。Oculyz-er测量角膜的厚度均较Sirius厚,Q值较Sirius偏负,临床使用时仍需慎重选择。 Objective To study the difference of corneal thickness measured by Oculyzer, Sirius and A mode ul- trasound,and to compare the difference of Q-values of the anterior comeal surface measured by Oculyzer and Sirius. Meth- ods Central corneal thickness ( CCT) in 50 eyes of 25 myopia patients were measured by Oculyzer, Sirius and A mode ul- trasound before refractive surgery, respectively. The difference of CCT was compared, and the correlation and concordance of three measurements were analyzed. Thinnest Corneal Thickness (TCT) and Q-values in 50 eyes were measured by Ocu- lyzer and Sirius . The difference of TCT and Q-values were compared, and the correlation and concordance of two measure- ments were analyzed. Results The mean CCT measured by Oculyzer, Sirius and A mode ultrasound were ( 546.70±31.94 ) μm, ( 535.96±31.96 ) μm, ( 541.00±32.60 ) μm, respectively. The CCT measured by Oculyzer was obvious higher than A mode ultrasound,the CCT measured by Sirius was obvious lower than that was measured by A mode ultra- sound, there were statistical differences ( both P 〈 0.05 ). There was a significant correlation in CCT measured by three measurements ( P 〈0.05 ),the Bland-Altman Plots displayed relatively good agreement limits of agreement between the three instruments. The mean TCT measured by Oculyzer and Sirius were (543.96±31.73)μm and(533.00±32. 19) μm. The TCT measured by Oculyzer was obvious higher than Sirius,there were statistical differences ( P 〈 0 05). There was a significant correlation in TCT measured by two measurements ( P 〈 0.05 ) ,the Bland-Airman Plots displayed rela- tively good agreement limits of agreement between the two instrtunents. The mean Q-value measured by Oculyzer and Sirius were -0. 1912±0. 1134 and -0. 1202±0. 1083. The Q-value measured by Oculyzer was obvious prolateness than Sirius, there were statistical differences( P 〈 0. 05 ). There was no significant correlation in Q-value measured by the two measure- ments ( P 〉0.05 ),but the Bland-Airman Plots displayed relatively good agreement limits of agreement between the two instruments. Conclusion Oculyzer and Sirius have good agreement with the gold standard A-mode ultrasound,the CCT and TCT measured by Oculyzer was thicker than that was measured by Sirius,bud the two instruments revealed a high correlation and good consistence; the anterior corneal sudace was more prolateness measured by Oculyzer than Sirius s The CCT, TCT and Q-value measured by two measurements have certain correlation, but also have the difference, these instruments should not be used interchangeably for CCT and TCT measurement in clinic.
