摘要
目的探讨血清人软骨糖蛋白-39(YKL-40)的水平与2型糖尿病(T2DM)及肥胖的关系。方法按体重指数(BMI)将42例初诊T2DM患者分为肥胖组(A组,20例,BMI≥25)和非肥胖组(B组,22例,BMI<25)。非糖尿病正常对照者46例亦分为肥胖组(C组,22例,BMI≥25)和非肥胖组(D组,24例,BMI<25)。ELISA法测定四组血清YKL-40浓度。结果 A组和B组血清YKL-40浓度高于C组和D组[(81.25±12.82)ng/ml和(79.82±13.47)ng/ml vs.(44.84±6.51)ng/ml和(40.98±8.74)ng/ml](P<0.01)。A、B组或C、D组之间血清YKL-40浓度比较差异均无统计学意义(P>0.05)。相关分析显示,血清YKL-40水平与空腹血糖、糖化血红蛋白呈正相关(P<0.01)。多元线性逐步回归分析表明,空腹血糖、糖化血红蛋白是血清YKL-40水平的独立影响因素。结论YKL-40与T2DM密切相关,可能参与了T2DM的发生、发展。
Objective To evaluate the correlation of serum YKL-40 with type 2 diabetes mellitus(T2DM)and obesity. Methods On the basis of body mass index(BMI), 42 newly diagnosed T2DM patients were divided into obese group(group A,20 cases,BMI≥25) and lean group(group B, 22 cases,BMI〈25). Another 46 subjects without T2DM were divided into obese group(group C, 22 cases,BMI≥25) and lean group(group D, 24 cases, BMI〈25). Serum concentration of YKL-40 was determined by ELISA. Results Serum levels of YKL-40 were higher in groups of A and B than those in groups of C and D [(81.25±12. 82) ng/ml and (79.82±13.47) ng/ml vs. (44. 84±6.51) ng/ml and (40. 98±8. 74) ng/ml] (P〈0. 01), which were not significantly different between groups of A and B or between groups of C and D(P〉0. 05). Correlation analysis showed that serum YKL-40 was positively correlated with fasting plasma glucose and HbAlc (P〈0. 01 ). Stepwise linear multiple regression analysis showed that fasting plasma glucose and HbAlc were the independent determinants of YKL-40. Conclusion YKL-40 may play a role in the pathogenesis of T2DM.
