MIPPO技术联合解剖锁定钢板治疗老年性股骨粗隆间骨折

The mini-invasive percutaneous plate osteosynthesis technique combined with proximal femur locking plate in treatment of intertrochanteric fracture in the elderly people

目的:观察微创经皮钢板内固定(MIPPO)联合解剖锁定钢板对老年性股骨粗隆间骨折的治疗效果。方法:回顾性分析2008年1月至2011年6月广州医学院第三附属医院收治的69例股骨粗隆间骨折患者的临床资料,根据治疗方式不同分为解剖锁定钢板组和动力髋螺钉组,观察两组患者的切口长度、出血量、手术时间、骨折愈合率、愈合时间、并发症及术后髋关节评分等指标。结果:69例患者术后均获得随访,平均随访时间18.4个月(6～28个月)解剖锁定钢板组患者切口长度、术中出血量、手术时间及术后髋关节功能评分等与动力髋螺钉组比较,差异有统计学意义(P<0.05)两组患者的骨折愈合率、愈合时间比较,差异无统计学意义(P>0.05)。结论:MIPPO联合解剖锁定钢板治疗老年性股骨粗隆间骨折疗效满意,是治疗该病的一种理想方法。

Objective： To evaluate the efficacy of mini-invasive percutaneous plate osteosynthesis （MIPPO） technique combined with proximal femur locking plate in treatment of intertrochanteric fracture in the elderly people. Methods： Between January 2008 and June 2011,69 patients with intertrochanteric fracture registered in the Thrid Affiliated Hospital of Guangzhou Medical College were retrospectively analyzed and divided into proximal femur locking plate （PF-LCP） and dynamic hip screw （DHS） groups. The incision length, amount of bleeding, surgical time, fracture healing rate, healing time, complication and hip joint scoring after surgery were observed in two groups. Results ： A total of 69 patients were followed up, with a mean following -up time being 18.4 months （ranging from 6 to 28 months）. There was difference in incision length, peri-operative bleeding amount,suigical time and hip joint scoring after surgery between PF-LCP and DHS groups （P 〈 0.05 ）. But, difference was not found in the fracture healing rate and healing time in two groups （ P 〉 0.05 ）. Conclusion： MIPPO combined with PF-LCP had a favorable efficacy and might be an idealized method in treatment of intertrochanteric fractures in elderly people.
