肺癌患者围术期心肌酶谱变化的临床分析

Clinical analysis of perioperative myocardial enzyme spectrum change in lung cancer patients

目的探讨肺癌患者围术期心肌酶谱变化的情况。方法回顾性分析本院手术治疗肺癌患者35例的临床资料。结果单纯手术组中,术后天门冬氨酸转氨酶(AST)、肌酸激酶(CK)、α-羟丁酸脱氢酶(α-HBDH)、肌钙蛋白I(cTnI)、肌红蛋白(Mb)指标水平明显比术前升高。新辅助化疗组中,术后AST、乳酸脱氢酶(LDH-L)、CK、肌酸激酶同工酶(CK-MB)、α-HBDH、cTnI、Mb等指标水平较术前明显升高。手术前后,新辅助化疗心肌酶谱指标α-HBDH、cTnI、LDH-L与CK明显高于单纯手术组患者。结论开胸手术和术前新辅助化疗均可以导致肺癌患者围术期心肌酶谱异常。

Objective To explore changes of the perioperative myocardial enzyme spectrum in lung cancer patients.Methods The clinical data of 35 patients who received surgical treatments in our hospital were analyzed retrospectively.Results In surgery-alone group,the index levels such as the postoperative aspartate aminotransferase（AST）,creatine kinase（CK）,alpha-hydroxybutyrate dehydrogenase（α-HBDH）,troponin I（cTnI） and myoglobin（Mb） were significantly improved compared with the preoperative.In neoadjuvant chemotherapy group,index levels such as postoperative AST,lactate dehydrogenase（LDH-L）,CK,creatine kinase isoenzyme（CK-MB）,α-HBDH and cTnI Mb were significantly increased compared with the preoperative.Before and after surgery,myocardial enzymes indicators including α-HBDH,cTnI,LDH-L and CK were significantly higher in neoadjuvant chemotherapy group than in the surgery-alone group.Conclusion Thoracic surgery and neoadjuvant chemotherapy before the surgery can lead to the abnormality of perioperative myocardial enzymes in lung cancer patients.
