单中心腹腔镜胆囊切除5100例临床体会

Clinical experience of single-port laparoscopic cholecystectomy in 5 100 cases

目的探讨如何正确掌握腹腔镜胆囊切除术（LC）手术适应证,合理运用手术技巧,预防并发症的临床体会。方法回顾分析我院1997年~2011年的5 100例LC的临床资料,对病例的选择、手术技巧的运用、并发症的预防进行的总结探讨。结果 5 100例LC术,中转开腹38例,中转率0.75%,术中胆总管损伤1例,术后出血1例。结论术前病例评估、术者熟练的手术技巧以及正确把握中转手术指针是LC术成功的关键。

Objective To explore the operative indications,surgical techniques and complication-prevention in laparoscopic cholecystectomy（LC）.Methods The clinical data of 5100 LC cases from 1997 to 2011 in our hospital were retrospectively analyzed.The selection of appropriate cases,the application of surgical techniques,and prevention of complications were summarized and discussed.Results Among 5100 cases of LC,only 38 cases were changed to open cholecystectomy with a conversion rate of 0.75%.There was one case of intraoperative common bile duct injury,and 1 case of postoperative bleeding.Conclusion Preoperative case assessment,skilled surgical techniques,and appropriate conversion to open operation are the keys to success of LC.
