高血压患者脉搏波传导速度与血压负荷关系的研究

Study on the relationship between brachial-ankle pulse wave velocity and blood pressure load in hypertensive patients

目的:探讨高血压患者臂踝脉搏波传导速度(BAPOWV)与血压负荷的关系。方法:入选原发性高血压患者348例,进行24 h动态血压监测和BAPWV测定,根据BAPWV的值,将其分为2组:BAPWV正常组(BAPWV<1400cm/s)121例,BAPWV升高组(BAPWV≥1400 cm/s)227例。比较2组患者年龄、性别构成比、血糖、血脂、血肌酐、血尿酸、血压及血压负荷。结果:单因素分析显示,BAPWV升高组患者的年龄、三酰甘油、胆固醇、低密度脂蛋白、血压及血压负荷均显著高于BAPWV正常组(P<0.05)。Logistic回归分析表明年龄、夜间收缩压负荷及夜间舒张压负荷是影响BAPWV的独立因素(P<0.05)。结论:高血压患者夜间收缩压负荷和舒张压负荷是影响BAPWV的独立因素。

Objective：To explore the relationship between brachial-ankle pulse wave velocity and blood pressure load in hypertensive patients. Methods：348 patients were enrolled in this study. Ambulatory blood pressure （ABP） and brachial-ankle pulse wave velocity （BAPWV） were measured. According to the value of BAPWV, all patients were divided into two groups ： BAPWV normal group （ bapwv 〈 1400 cm/s, n = 121 ） and BAPWV elevated group （bapwv ≥ 1400 cm/s, n = 227）. The patients＇ age, gender ratio, fasting blood glucose （FBG）, triglyceride （TG）, cholesterol （CHO）, low density lipoprotein cholesterol （LDL）, high density lipo- protein cholesterol （HDL）, creatinine （Cr）, uric acid （UA）, blood pressure level and blood pressure load were measured. Results：Univariate analysies revealed that age, TG, CHO, LDL, blood pressure and blood pressure load in BAPWV elevated group were significantly higher than those in BAPWV normal group （P 〈 0.05 ）. Logistic multivariate regression analysis indicated that age, nighttime systolic blood pressure load and nighttime diastolic blood pressure load were independent influencing factors of BAPWV. Conclusion：nighttime systolic blood pressure load and nighttime diastolic blood pressure load of hypertensive patients were independ- ent influencing factors of BAPWV.
