摘要
目的:对结肠癌患者血清中肿瘤相关抗原(TAA)Imp1、p62、Koc、p53、c-myc的抗体进行联合检测,探讨其对结肠癌的诊断价值。方法:用酶联免疫吸附试验(ELISA)对64例结肠癌患者血清和58例正常对照血清中的5种肿瘤相关抗原的抗体进行检测;用电化学发光免疫分析方法(ECLIA)对血清标本的癌胚抗原(CEA)含量进行测定;对抗体检测结果进行灵敏度和特异性分析。结果:每种肿瘤相关抗原的检测结果单独进行判断时,灵敏度都偏低,介于15.6%至23.4%。对5种抗原进行不同组合,结果随着抗原种类的增加,灵敏度随之增加,5种抗原联合检测的敏感度达到了60.9%,特异度达到了89.7%。用CEA和抗TA-As抗体联合检测,其阳性率则达到了82.8%。结论:5种TAAs抗体联合检测结肠癌,具有较高的真实性和诊断质量,为结肠癌的早期诊断和高危人群的筛选,及降低CEA阴性的结肠癌患者漏诊率提供了一种新方法。
Objective:To investigate the diagnostic value of the serum antibodies to five tumor-associated antigens(TAAs) including Imp1,p62,Koc,p53 and c-myc on colon cancer.Methods: Enzyme-linked immunosorbent assay(ELISA) was used to detect antibodies against these five TAAs in 64 sera from patients with colon cancer and 58 sera from normal individuals.Electro chemiluminescence immunoassay(ECLIA) was used to measure the serum carcino-embryonic-antigen(CEA).Results: Detection result of antibody to any individual ATT showed low sensitivity in the range of 15.6%~23.4%.With the successive addition of TAAs to a final total of five antigens,there was a stepwise increase of positive antibody reactions,reaching a sensitivity of 60.9% and a specificity of 89.7% in colon cancer.When CEA and anti-TAAs were used together as markers in colon cancer detection,the diagnostic sensitivity could be raised to 82.8%.Conclusion: This study shows that a panel of five TAAs can enhance antibody detection for diagnosis of colon cancer,which may be used as a screening tool for population at high risk in colon cancer,and also a routine test in clinical practice especially to patients with negative CEA.
