心脏瓣膜病围手术期ST段改变的临床意义

The value of ST segment changes during perioperation in valvular heart disease

目的探讨瓣膜病围手术期心电图ST段动态变化的临床意义。方法回顾性分析比较354例瓣膜病患者行瓣膜置换术前与术后1周内不同时期12导联心电图ST段动态变化。结果与术前比较,ST段抬高的导联数在术后第1天开始明显增加(p<0.01),之后逐渐回落,第6天基本恢复至术前水平;ST段下移的导联数在术后亦开始减少,至第7天时下移的导联数较术前明显减少。结论了解瓣膜病患者瓣膜置换术围术期心电图ST段变化的规律,对于疗效评价及愈后的判断有重要的临床意义。

Objective To discuss the dynamic changes of ST segment during perioperation in heart valve replacement. Methods A retrospective study was carried out to review experience. 354 cases underwent heart valve replacement. The changes of ST segment in a week postoperation were compared with those before operation. Results The number of leads with ST segment elevation starting from the first day after operation increased significantly than that before operation （p〈0.01）, and then it became down gradually. It got back to the level of before operation on the sixth day postoperation. The number of leads with ST segment depression also decreased after operation. On the seventh day after operation, the number of leads with ST segment depression decreased significantly than that before operation. Conchmion It will be benefit to realize the changes of ST segment during perioperation in heart valve replacement.
