摘要 目的:探讨黏附分子L1在胃肠道间质瘤(GIST)中的表达情况及其临床意义。方法:采用免疫组织化学法和RT-PCR检测66例GIST、10例平滑肌肿瘤、6例神经鞘瘤及14例正常肠壁组织中L1的表达情况,并分析其临床意义。结果:胃肠道间质瘤中L1蛋白及mRNA的表达率分别为57.6%(38/66)和83.3%(30/36),差异有统计学意义(P<0.05);L1在不同侵袭危险性GIST中的表达差异有统计学意义(P<0.05),但在不同性别、年龄、肿瘤部位、组织学类型中的表达差异无统计学意义(P>0.05)。L1在GIST中的表达明显高于其在平滑肌肿瘤及正常肠组织中的表达(P<0.01),与其在神经鞘瘤中的表达差异无统计学意义(P>0.05)。结论:L1表达在GIST发生发展过程中起促进作用,L1可作为GIST的诊断和鉴别诊断指标。 Objective: To investigate the expression of the adhesion molecule L1 in gastrointestinal stromal tumors (GIST), and the clinical significance thereof. Methods: The expression leyels of L1 were detected in tissues of 66 samples of GIST, 10 samples of smooth muscle tumors, 6 samples of schwannoma and 14 samples of normal bowel by immunohistochem- istry and RT-PCR, and their clinical significances were analyzed~ Results: The expression rates of L1 protein and mRNA in GIST were 57.6% (38/66) and 83.3% (30/36), which were significantly different between two groups (P 〈 0.05). The L1 expres- sion was significantly different in the different aggressive risk of GIST (P 〈 0.05), but there was no significant difference in dif- ferent age, gender, tumor locus and tissue type groups (P 〉 0.05). The expression of L1 was significantly higher in GIST than that of smooth muscle tumor and normal bowel tissues (P 〈 0.01). There was no significant difference in the expression of L1 between GIST group and schwannoma groupss (P 〉 0.05). Conclusion: The expression of L1 may play an important role in tu- morigenesis and progression of GIST, which suggest that L1 can be applied to diagnosis or differential diagnosis of GIST.
