椎间盘镜辅助下经皮椎弓根螺钉内固定治疗胸腰椎骨折

Percutaneous Pedicle Screw Internal Fixation under Diskoscope in the Treatment of Thoracolumbar Fractures

目的:探讨椎间盘镜辅助下经皮椎弓根螺钉内固定治疗胸腰椎骨折的疗效。方法:2009年2月-2011年7月,采用椎间盘镜辅助下经皮椎弓根螺钉内固定治疗胸腰椎骨折18例(微创组),并与常规的开放手术组(15例)相比较。结果:两组病例的伤椎前后缘高度百分比、后凸Cobb’s角术前术后相比差异有统计学意义(P<0.01),两组间术后影像学观察指标比较差异无统计学意义(P>0.05)。微创组较常规的开放组的切口长度、术中出血量、住院时间明显减少,(P<0.01),但手术时间两组比较差异无统计学意义(P>0.05)。结论:椎间盘镜辅助下经皮椎弓根螺钉技术具有创伤小、出血少、术后恢复快等优点,是治疗胸腰椎骨折的有效方法。

Objective：To detect the feasibility of percutaneous pedicle screw internal fixation under diskoscope in the treatment of thoracolumbar fractures. Methods： Eighteen patients with thoracolumbar fractures were treated by percutaneous pedicle screw internal fixation under diskoscope （mini--trauma group）, and 15 patients were treated by traditional open operation （traditional group） from Feb. 2009 to Jul. 2011. Results ; The postoperative vertebral height and Cobb＇s angle were significantly increased in both groups （P〈0.01）. The incision size, the blood volume of operation, and the hospital stay in mini-trauma group were significantly less than traditional group （P〈0.01）. But the operative time and indexes of postoperative iconography were not different in the two groups （P〉0.05）. Conclusion：Percutaneous pedicle screw internal fixation under diskoscope is effective for treating thoracolumbar fractures, which has some advantages such as mini-traumas, less blood lose and postoperative recovery quickly.
