椎间盘镜辅助下经皮椎弓根螺钉内固定治疗胸腰椎骨折 被引量:2

Percutaneous Pedicle Screw Internal Fixation under Diskoscope in the Treatment of Thoracolumbar Fractures

摘要 目的:探讨椎间盘镜辅助下经皮椎弓根螺钉内固定治疗胸腰椎骨折的疗效。方法:2009年2月-2011年7月,采用椎间盘镜辅助下经皮椎弓根螺钉内固定治疗胸腰椎骨折18例(微创组),并与常规的开放手术组(15例)相比较。结果:两组病例的伤椎前后缘高度百分比、后凸Cobb’s角术前术后相比差异有统计学意义(P<0.01),两组间术后影像学观察指标比较差异无统计学意义(P>0.05)。微创组较常规的开放组的切口长度、术中出血量、住院时间明显减少,(P<0.01),但手术时间两组比较差异无统计学意义(P>0.05)。结论:椎间盘镜辅助下经皮椎弓根螺钉技术具有创伤小、出血少、术后恢复快等优点,是治疗胸腰椎骨折的有效方法。 Objective:To detect the feasibility of percutaneous pedicle screw internal fixation under diskoscope in the treatment of thoracolumbar fractures. Methods: Eighteen patients with thoracolumbar fractures were treated by percutaneous pedicle screw internal fixation under diskoscope (mini--trauma group), and 15 patients were treated by traditional open operation (traditional group) from Feb. 2009 to Jul. 2011. Results ; The postoperative vertebral height and Cobb's angle were significantly increased in both groups (P〈0.01). The incision size, the blood volume of operation, and the hospital stay in mini-trauma group were significantly less than traditional group (P〈0.01). But the operative time and indexes of postoperative iconography were not different in the two groups (P〉0.05). Conclusion:Percutaneous pedicle screw internal fixation under diskoscope is effective for treating thoracolumbar fractures, which has some advantages such as mini-traumas, less blood lose and postoperative recovery quickly.
