乳腺浸润性导管癌ER、PR、Her-2和nm23表达的相关性及临床病理学意义 被引量:2

Expression of ER 、PR、 Her-2 and nm23 in invasive ductal carcinoma of breast:their correlation and clinicopathological significance

摘要 目的:探讨癌基因Her-2、抑癌基因nm23和雌激素受体(ER)、孕激素受体(PR)在乳腺癌中的表达及临床病理学意义。方法:应用免疫组化SP法检测68例乳腺浸润性导管癌中ER、PR、Her-2和nm23的表达情况,分析其与肿瘤大小、肿瘤TNM分期、淋巴结转移等临床病理指标的相关性。结果:(1)ER表达与癌组织分化、肿瘤TNM分期及肿瘤大小呈负相关,与患者年龄及淋巴结转移无显著相关性;(2)PR表达与癌组织分化、肿瘤TNM分期呈负相关,与肿瘤大小、患者年龄及淋巴结转移无显著相关性;(3)Her-2表达与肿瘤TNM分期、组织学分级、肿瘤大小及淋巴结转移呈正相关,与患者年龄无显著相关性;(4)乳腺癌I期nm23表达阳性率及阳性强度明显高于II、III期乳腺癌,nm23表达与组织学分级、淋巴结转移呈负相关,与肿瘤大小及患者年龄无显著相关性;(5)ER与PR表达呈正相关,nm23表达与ER、PR表达呈正相关;与Her-2表达呈负相关;癌组织中Her-2与ER表达呈负相关,与PR表达无显著相关性。结论:ER、PR、Her-2和nm23的联合检测能较好地反映乳腺癌病理生物学特征。 Objective: To investigate the expression and significance of estrogen receptor (ER) ,progestogen receptor (PR) ,Her-2 and nm23 protein in breast Carcinoma. Methods :68 cases of surgical specimens from infiltrating breast Canc- er were investigated by S-P immunohistochemical Staining for ER,PR,Her-2 oncogene and nm23 gene protein. Results: ( 1 ) The positive rate of ER was 55.8%, which was negatively correlated with histological grade,TNM stage and tumor size. but that did not relate to age and metastasis; (2)The positive rate of PR was 58.5% ,did not relate to tumor size,histological types, age and metastasis; ( 3 ) The positive rate of Her-2 was 63.2% ,The expression of Her-2 was positively correlated with histological grade, TNMstage, tumor size and lymph node metastasis, but did not realte to histological types and age;(4)The positive rate of nm23was 69.1% ,which was negatively correlated with histological grade,TNM stage and lymph node me- tastasis, and did not relate tO tumor size and histological types ; (5) The expression of ER was positively correlated with that PR,so did nm23 with ER and PR, respectively that negatively correlated with Her-2 expression. The expression of Her-2 was negatively correlated with ER expression,but did not relate to PR expression. Conclusion:The expression of ER,PR, Her-2 and nm23 in brest cancer cells reflects the Dathobioloic features of the cancer.
