摘要
目的应用奥马哈系统对出院2周内的脑卒中患者居家康复护理健康问题进行评估,为针对性护理干预措施提供理论依据。方法选取25例脑卒中患者在出院后1～2周进行社区家庭访视,社区护士应用奥马哈系统评估患者个人的、现存的健康问题。结果脑卒中患者健康问题分布在奥马哈系统的四个领域,其中出现问题最多的是营养、处方药物治疗方案、住宅、神经-肌肉-骨骼功能等方面的问题。结论奥马哈系统能够全面地评估脑卒中患者出现的健康问题,为社区护士进行针对性护理干预提供理论依据。
Objective To assess the health problems encountered by stroke clients in community rehabilitation nursing so as to provide theoretical references for corresponding nursing interventions.Methods A total of 25 stroke clients were recruited,and provided with home visit 1~2 weeks after hospital discharge.Omaha system was employed to assess the health problems among the stroke clients.Results The health problems mainly involved four domains of Omaha System,among which nutrition,medication regimen,residence and neuro-musculo-skeletal function were the most problematic.Conclusions Omaha System can provide a comprehensive assessment for the stroke clients and provide a theoretical reference for community rehabilitation nursing.
