复方甘草酸苷联合阿昔洛韦、神经妥乐平治疗带状疱疹神经痛的疗效

Effect of compound glycyrrhizin combing with acyclovir and neurotropin on postherpetic neuralgia

目的:观察复方甘草酸苷联合阿昔洛韦、神经妥乐平治疗带状疱疹后遗神经痛(postherpeticneuralgia,PHN)的临床效果。方法:选取门诊2006年5月～2009年3月收治的130例PHN患者,按照随机数字表法随机分为观察组(复方甘草酸苷联合阿昔洛韦、神经妥乐平治疗组)和对照组(氨酚曲马多片联合卡马西平治疗组)各65例,两组患者连续治疗8周(2个疗程)后比较临床疗效及不良反应情况。结果:(1)观察组基本治愈36例,显效21例,总有效率为87.7%;对照组基本治愈28例,显效14例,总有效率为64.6%。两组患者总有效率比较差异有统计学意义(χ2=17.546,P<0.01)。(2)两组患者治疗前VAS评分相似,治疗2周后观察组改善较对照组明显(P<0.05);治疗4周、8周后观察组改善较对照组更加显著(P<0.01)。(3)两组患者治疗2、4周时疼痛缓解时间比较差异无明显统计学意义(P>0.05);治疗2、4周时镇痛维持时间差异均有统计学意义(P<0.01)。(4)观察组出现面色潮红1例,恶心1例,无血清钾下降、血压升高、休克等不良反应;对照组出现头晕2例。结论:复方甘草酸苷联合阿昔洛韦、神经妥乐平治疗带状疱疹后遗神经痛(PHN)效果理想,可以显著提高疼痛缓解率、延长疼痛缓解时间、提高治疗效果,且临床用药安全,值得推广应用。

Objective： To observe effects of compound glycyrrhizin combing with acyclovir and neurotropin on postherpetic neuralgia（PHN）.Methods： A total of 130 PHN patients in outpatients were selected from 2006 May to 2009 March,and were randomly divided into the observation group（Compound glycyrrhizin in combination with acyclovir and neurotropin） and control group（paracetamol and tramadol tablets combined with carbamazepine treatment） by half.After 8 weeks（2 consecutive course）,effects and side effects were observed and compared.Results：Thirty six cases were cured in observation group,and 21 cases was effective,with total effective rate of 87.7%;28 cases were cured in control group,14 cases was effective,with total effective rate of 64.6%.There was significant difference in efficacy between two groups（χ2= 17.546,P〈0.01）.Before treatment VAS scores were similar in two groups.But patients in observation group showed more significant improvement after 2,4 and 8 weeks（P〈0.05 or P〈0.01）.There was no significant difference in pain relief time after 2 or 4 weeks（P〉0.05）;but the difference in analgesic duration was significant both after 2 and 4 weeks（P〈0.01）.0ne case in observation group had facial flushing,1 had nausea,but no decreased serum potassium,elevated blood pressure,shock or other adverse reactions occurred;there was 2 cases in control group had dizziness.Conclusion： Combination treatment of compound glycyrrhizin,acyclovir and neurotropin can increase pain relief rate,prolong relief time and improve effect.It is safe and effective in treating PHN,and is worthy of clinical application.
