海口地区慢性乙型肝炎568例HBeAg阴性与阳性临床比较分析

Comparison of clinical characteristics of HBeAg-negative and HBeAg-positive chronic hepatitis B in Haikou

目的:通过比较慢性乙型肝炎患者中HBeAg阴性和HBeAg阳性两类临床相关因素的异同,了解海口地区近年慢性乙型肝炎的流行病学特点及临床特征。方法:采用描述流行病学方法,对海南医学院附属医院2003年至2011年568例住院慢性乙型肝炎的发病情况进行分析。结果:568例慢性乙型肝炎患者,HBeAg阴性374例(65.8%),HBeAg阳性共194例(占34.2%);平均年龄(40.44±14.853)岁,男性发病明显多于女性(4∶8∶1);HBeAg阴性组发病年龄高于阳性组;HBeAg阴性组肝硬化、肝癌所占比例明显高于HBeAg阳性组;HBeAg阳性组HBV DNA载量总体上高于HBeAg阴性组。结论:目前海口地区慢性乙型肝炎以HBeAg阴性者占多数,HBeAg阴性患者平均年龄高、预后差,是我地区今后防治的重点。

Objective： To explore the epidemiological characteristics and clinical features of Chronic hepatitis B （CHB） in Haikou area, and to compare the clinical characteristics of HBeAg-negative and HBeAg-positive chronic hepatitis B. Methods：A total of 586 cases of CHB that admitted to the Affiliated Hospital of Hainan Medical College from 2003 to 2011 were analyzed by descriptive epidemiological method. Results. Of the 568 cases, 374 （65.8%） were HBeAg-negative and 194 （34.2%） were HBeAg-positire. The average ages of patients were （40.440 ±14. 853） years old, and the prevalence of CHB among males was higher than that among females （4.8.1）; Compared with HBeAg-positive group, HBeAg-negative group showed elder average age, higher constituent ratio of hepatocirrhosis,hepatocellular carcinoma; while HBeAg-positive group showed a higher serum HBV DNA levels than HBeAg-negative group. Conclusions： HBeAg-negative CHB is still the predominant form of CHB in Haikou, and average age of these patients is old, prognosis is poor, thus they are the focus of treatment and prevention measures in Haikou.
