摘要
目的:探讨淋巴结转移率(LNR)对Ⅲ期结直肠癌患者预后的影响。方法:回顾性分析我院255例Ⅲ期结直肠癌患者的临床病理资料和随访资料。结果:255例患者共计检出淋巴结2 952枚;转移淋巴结490枚;清扫的淋巴结检出数≥12枚与<12枚的Ⅲ期患者的5年生存率分别为58.7%和28.7%,差异有统计学意义(P<0.01)。不同LNR的Ⅲ期结直肠癌患者5年生存率差异有统计学意义(P<0.01)。结论:淋巴结检出数对Ⅲ期结直肠癌患者预后的影响更为明显。LNR是影响Ⅲ期结直肠癌的预后的重要因素,用其评估患者预后有效补充了TNM分期不足。
Objective:To evaluate the prognostic significance of the lymph node ratio(LNR) in patients with stage Ⅲ colorectal cancer.Methods:Clinicopathological data together with the information obtained by follow-up were reviewed in 255 patients with stage Ⅲ colorectal cancer undergone surgical treatment in our hospital.Results:The total number of retrieved lymph nodes were 2 952 in which 490 were metastic lymph nodes in the 255 cases.The 5-year survival rate for retrieval of the lymph nodes greater than 12 or less than 12 was 58.7% and 28.7%,respectively,which suggested statistical difference(P〈0.01),and the 5-year survival rate was also statistically different in stage Ⅲ colorectal cancer with dissimilar LNR(P〈0.01).Conclusion:The number of lymph nodes retrieved can be strongly associated with the prognosis in patients with stage Ⅲ colorectal cancer,suggesting that LNR has prognostic implications and may make up for the deficiency of INM staging in estimation of the survival rate of this tumor.
