2型糖尿病合并肺结核患者细胞免疫功能的影响因素

THE INVESTIGATION OF THE INFLUENCE FACTORS OF CELL IMMUNITY FUNCTION IN PATIENTS WITH PULMONARY TB ACCOMPANIED BY T2DM

目的探讨2型糖尿病(T2DM)合并肺结核(PTB)患者糖化血红蛋白(HbAlc)和血清白蛋白(ALB)对细胞免疫功能的影响,为临床治疗提供依据。方法随机选择87例新入院T2DM合并PTB患者做为研究对象,用流式细胞仪测定T细胞亚群,并分别按ALB和HbAlc是否正常分组,比较T细胞亚群各值。结果 TP与NK,ALB与NK、CD3+,HbAlc与CD8+有一定的正相关性,p<0.05。低蛋白血症组的外周血NK细胞、CD3+明显低于正常组,均p<0.05,CD8+明显高于于正常组,p<0.01。高血糖组的外周血CD8+水平明显高于理想组,p<0.05。结论低蛋白血症和长期高血糖状态会损害T2DM合并PTB患者的细胞免疫功能。改善蛋白质营养状态、严格控制血糖水平有助于改善机体细胞免疫功能,促进结核病的转归。

Objective To investigation the cell immunity function＇s change in patients with pulmonary tubereulosis（PTB） accompanied by type 2 diabetes mellitus （T2DM） , and its relationship with Glycated hemoglobin （HbAlc） and Serum albumin （ ALB ） , so as to provide references for therapy. Methods Randomly selected 87 new inpatients with PTB accompanied by T21）M for study, checked the subgroup of T cells in them by flow cell sorter. All the patients were respectively divided into two groups according the level of HbAlc and ALB is normal or not. Compare the values of different subgroups T cells. Results Serum Total protein（TP） ex- hibited a positive linear correlation with NK,the same as ALB with NK,CD~＋ ,and HbAlc with CDs＋ ,p 〈0.05. The level of NK and CD; in group with hypoalbuminemia were significant lower than those normal, p 〈 0. 05. The level of CDs＋ was significant higher than those normal, p 〈 O. 05. CD3 is significantly higher in hyperglycemia group than ideal control group p 〈 0. 05. Conclusion The cell immunity function in patients with PTB accompanied by T2DM would be affected by Hypoalbuminemia and Hyperglycemia. Improving the nutrition of albumin and strictly controlling the lever of blood glucose is benefit to improve the cell immunity function and promote the outcome of TB.
