饮食干预对胃癌毕Ⅱ式根治术后患者生活质量及并发症的影响

Influence of Dietary Interventions on Gastric Cancer Patients' Quality of Life and Complications after Billroth Ⅱ Gastrectomy

目的探讨饮食干预对毕Ⅱ式胃癌根治术后患者生活质量的影响。方法便利抽样法选取2009年6月至2010年12月在桂林医学院附属医院行胃癌毕Ⅱ式根治术的患者60例为研究对象,按随机数字表法将其分为观察组和对照组,每组30例。对照组患者根据胃癌毕Ⅱ式根治术后饮食原则给予常规饮食指导,观察组在此基础上根据患者的个体情况,与营养师合作按照循序渐进原则进行个体化指导及跟踪督查、指导。分别于术后6个月、1年采用胃癌患者生活质量问卷(quality oflife questionnaire of stomach 22,QLQ-STO22)评定两组患者的生活质量并统计并发症的发生情况。结果观察组患者总体生活质量明显高于对照组,差异有统计学意义(P<0.01)。两组患者术后均有进餐后腹胀、食物反流、倾倒综合征等并发症发生;术后6个月,观察组患者的并发症发生率均低于对照组,差异有统计学意义(P<0.05)。结论个体化饮食干预可改变患者的不良生活方式,降低并发症的发生率,提高患者的生活质量。

Objective To investigate the influence of dietary interventions on the gastric cancer patients＇ quality of life after Billroth 1I gastrectomy. Methods From June 2009 to December 2010,60 patients undergoing Billroth II gastrectomy in the hospital were randomly divided into interventional group（n=30） and control group（n=30）. The patients of the control group received conventional dietary guidance,and the interventional group used the project of individual dietary guidance in accordance with the principle of grad- ual and orderly progress at the suggestion of nutritionists. The patients＇ quality of life and the incidences of complications were evaluated at six months and one year after gastrectomy separately using quality of life questionnaire of stomach 22（QLQ-STO22）. Results The patients＇ overall QLQ-STO22 level in the inter- ventional group was significantly higher than that of the control group （P〈0.01）. The patients in both groups suffered from bloating after meals, gastro-esophageal reflux, dumping syndrome and other complications. The incidence of complications in 6 months after operation was significantly lower in the observational group than that in the control group（P〈0.05）. Conclusion Individual dietary interventions can change patients＇ unhealthy lifestyle, reduce the complication rate and improve their quality of life after gastrectomy.
