米力农治疗慢性充血性心力衰竭的疗效观察

Response to Milrinone Treatment in Patients with Chronic Congestive Heart Failure

为验证米力农治疗慢性充血性心力衰竭（CHF）患者的疗效，采用随机双盲法将30例慢性充血性心衰患者，分为米力农治疗组和安慰剂对照组进行观察。结果：米力农组每博量由38.5±5．34增加到64．93±6．46ml，心排血量由3．39±0．42增加到5．28±0．55L／min，心脏指数由2．01±0．42增加到3．21±0．57L／min2，射血分数由0．23±0．05增加到0．35±0．06治疗前后均有非常显著性差异，（P均＜0．001）。安慰剂对血流动力学指标无明显影响。米力农治疗组治疗后心功能改善11例（11／15例，73．3％），而安慰剂组仅4例（4／15例，26．7％），两组间有显著性差异（P＜0．05）。以上显示，米力农可显著改善慢性充血性心衰患者的血流动力学状况和心功能。

The objective of this study was to examine the efficacy of milrinone in chronic congestive heart failure. In this randomized, double-blind, placebo-controlled trial, 30 patients with class, Ⅲ or Ⅳ chronic congestive heart failure were randomly assigned to receive milrinone (n =15) or placebo (n =15). The results showed that in the milrinone group, stroke voIume increased from 38.50±5. 34 to 64. 93±6. 46ml(P<0. 001),cardiac output inereased from 3. 39±0. 42 to 5. 28±0. 55 L/min (P<0. 001), cardiac index inereased from 2.01±0. 42 to 3. 21 ± 0. 57/min/m2 (P<0.001) and ejection fraction increased from 0. 23 ± 0. 05 to 0. 35 ± 0. 06 (P<0. 001). In the placebo group, no hemodynamic effects were found. Milrinone favourablly influenced the symptoms and heart function in 11 /15(73. 3% ) pateints. In the pacebo group, only 4/15(26. 7% ) patients had such response (P<0. 05). These data indicate that milrinone is effective for imporving the hemodynamics status and heart function in patients with chronic heart failure.
