诺和灵、血栓通及奥扎格雷钠联合治疗2型糖尿病并发急性脑梗死疗效观察

Effect of novolin combined with Xueshuantong and sodium ozagrel on patients with type 2 diabetes mellitus complicated acute cerebral infarction

目的评价诺和灵、血栓通和奥扎格雷钠联合治疗2型糖尿病(T2DM)并发急性脑梗死(ACI)的临床疗效。方法 90例T2DM并发ACI患者分为3组,A组患者采用诺和灵、血栓通和奥扎格雷钠联合治疗,B组患者采用口服降糖药、血栓通和奥扎格雷钠联合治疗,C组患者采用口服降糖药和丹参注射液治疗;对3组疗效进行比较。结果 3组患者治疗后神经功能缺损程度评分显著低于治疗前(P<0.05);治疗后A组患者神经功能缺损程度评分显著低于B、C组(P<0.05)。治疗后3组患者空腹血糖、血小板计数及凝血酶原时间与治疗前比较差异均无统计学意义(P>0.05);除C组外,A、B组患者治疗后纤维蛋白原水平低于治疗前(P<0.05);治疗后A、B组患者纤维蛋白原水平低于C组(P<0.05);A组总有效率显著高于B、C组(P<0.05),而B组与C组比较差异无统计学意义(P>0.05)。A组患者肺部及尿路感染率低于B、C组(P<0.05)。结论诺和灵、血栓通和奥扎格雷钠联合治疗T2DM并发ACI疗效好,可有效改善患者神经功能缺损,减少并发症。

Objective To evaluate the effect of novolin combined with Xueshuantong and sodium ozagrel on patients with type 2 diabetes mellitus （ T2DM ） complicated acute cerebral infarction （ ACI ）. Methods Ninety patients with T2DM complicated ACI were divided into three groups. The patients in group A were treated with novolin, Xueshuantong and sodium ozagrel;the patients in group B were treated with oral antidiabetic drug, Xueshuantong and sodium ozagrel;and the patients in group B were treated with oral antidiabetic drug and salviae miltiorrhizae injection. The effects were compared among three groups. Results The score of neurologic impairment after treatment was significantly than that before treatment in three groups （ P 〈 0.05 ）. After treatment, the score of neurologic impairment in group A was significantly lower than that in group B and C （ P 〈 0.05 ）. There was no significant difference in the fasting blood glucose, blood platelets count and prothrombin time be- tween pretreatment and posttreatment in three groups （ P 〉 0.05 ）. Except group C, the levels of fibrinogen in Group A and B af- ter treatment were lower than those before treatment （ P 〈 0.05 ）. The total effective rate in group A was significantly higher than that in group B and C （P 〈 0.05 ）, but there was no significant difference in the total effective rate between group B and group C （ P 〉 0.05 ）. The rates of pulmonary and urinary tract infection were significantly lower than those in group B and C （ P 〈 0.05 ）. Conclusion Novolin combined with Xueshuantong and sodium ozagrel on patients with T2DM complicated ACI can obtain good effect, effectively improve neurologic impairment, and decrease complication.
